<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT -
<br />BLACKTHORN CORPORATE PtS:RK -SANITARY SEWER AND WATER MAIN
<br />RELOCATION - HERRMAN & GOETZ -PROJECT NO. 103-029 (BLACKTHORN
<br />TIF/SEWAGE WORKS)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Final Change Order
<br />on behalf of Herrman & Goetz, 225 South Lafayette, South Bend, Indiana, indicating that
<br />the contract amount be increased by $11,773.00 for a new contract sum including this
<br />Change Order in the amount of $130,730.00. Additionally submitted was the Project
<br />Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Inks,
<br />' seconded by Mr. Gilot and carried, the Final Change Order and the Project Completion
<br />Affidavit were approved subject to the filing of the appropriate three-year Maintenance
<br />Bond.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT -DEMOLITION OF FORMER
<br />SEARS REPAIR CENTER -UNSUITABLE FILL REMOVAL - RITSCHARD
<br />BROTHERS -PROJECT NO 106-025A (COIT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Ritschard Brothers, 1204 West Sample, South Bend, Indiana, for
<br />the above referred to project, indicating a final cost of $74,239.95. Mr. Inks noted a
<br />memorandum dated September 29, 2006 from Board Attorney Thomas Bodnar, and a
<br />memorandum from Engineer Rob Nichols, Engineering, were submitted to the Board
<br />describing unforeseeable circumstances in conjunction with this project. Upon a motion
<br />made by Mr. Inks, seconded by Mr. Gilot, and carried, the Project Completion Affidavit
<br />was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT -DEMOLITION OF FORMER

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Ritschard Brothers, 1204 West Sample, South Bend, Indiana, for
<br />' the above referred to project, indicating a final cost of $62,234.00. Upon a motion made
<br />by Mr. Inks, seconded by Mr. Gilot, and carried, the Project Completion Affidavit was
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT - STUDEBAKER DEMOLITION
<br />AREA "A" -PHASE II -FORMER HUCKINS TOOL AND DIE BUILDING -
<br />RITSCHARD BROTHERS - PROJECT NO. 106-029
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Ritschard Brothers, 1204 West Sample, South Bend, Indiana, for
<br />the above referred to project, indicating a final cost of $41,961.00. Upon a motion made
<br />by Mr. Inks, seconded by Mr. Gilot, and carried, the Project Completion Affidavit was
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT -MORRIS PERFORMING ARTS
<br />CENTER. ALLEY DRAINAGE -UNDERGROUND SERVICES -PROJECT NO. 106-

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Underground Services, 2915 Home Street, Mishawaka, Indiana,
<br />for the above referred to project, indicating a final cost of $15,865.00. Upon a motion
<br />made by Mr. Inks, seconded by Mr. Gilot, and carried, the Project Completion Affidavit
<br />was approved.
