Please note that this ESTIMATED COST should be considered a Project Maximum, <br />which PES will not exceed without prior approval. However, should the scope of <br />services require modification (i.e., additional borings) based upon field conditions and/or <br />circumstances that would not have otherwise been anticipated, PES will notify the client <br />of such "changed conditions" and resultant cost implications to the project. <br />Should Geoprobe services require only 1/2-day for completion of boring/temporary wells, <br />a rate adjustment will be reflected within the invoice. Should PES fail to encounter <br />groundwater within the upper 30 feet of the soil profile, the invoice will reflect the fact <br />that groundwater samples were not submitted to the laboratory for analysis (credits <br />applied). <br />LIMITATIONS/QUALIFICATIONS <br />The sampling and testing of soil, groundwater and/or other materials not specifically <br />identified in this proposal is beyond the scope of services for this project. Cost estimates <br />for our proposed services have been prepared based on the following assumptions: <br />• PES will have unlimited access to the site during daylight hours; <br />• No subsurface obstructions (buried utilities or fill debris) will interfere with our work; <br />• Sufficient access will be provided for Geoprobe locations; <br />• Inclement weather will not interfere with the completion of our field work; <br />• Soil cuttings will be returned to the boring from which they were generated, with the <br />remainder of the boring filled with bentonite pellets to land surface. <br />This Phase II Environmental Site Assessment should be considered as a "tool" in the <br />evaluation of the subject site relative to the potential presence of contaminants, and <br />does not necessarily fulfill regulatory requirements of the Indiana Department of <br />Environmental Management (IDEM) for delineation of contaminants identified within soil <br />or groundwater. Should contamination be identified through completion of this Phase II <br />Environmental Site Assessment, additional assessment may be necessary. <br />AUTHORIZATION <br />Phifer Environmental Services, Inc. appreciates the opportunity to be of service on this <br />project.
