Tuition Reimbursement: Qualifying officers and sworn members of the South <br />Bend Police Department participating in the tuition reimbursement program shall be entitled <br />to the following maximum dollar amounts for any academic year for course tuition only: <br />('nurse Grade Maximum Amount Reimbursed b itv: <br />A 50 % of tuition costs only up to maximum of $1,000 <br />B 35 % of tuition costs only up to maximum of $ 750 <br />C 25 % of tuition costs only up to maximum of $ 500 <br />All governing regulations must be complied with in order to qualify for said tuition <br />reimbursement. <br />K. QReration Safe Neighborhood Reimbursement: Qualifying officers and sworn <br />members of the South Bend Police Department who establish legal residence within <br />designated areas of the City of South Bend limits and reside on a continuous permanent <br />basis within such designated areas with the city for the period of time he or she is seeking <br />reimbursement shall be entitled to $ 1,500.00 of his or her annual mortgage or rental <br />amount. Qualifying officers and sworn members shall be entitled tobe reimbursed by the <br />City on a quarterly basis. All governing regulations must be complied with in order to <br />qualify for said reimbursement. <br />L. Life Insurance: Effective January 1, 2000, the City of South Bend shall <br />increase the term life insurance coverage on all sworn members and officers from $ 15,000 <br />to $ 20,000. <br />M. Health Insurance: The City shall maintain a comprehensive major medical <br />insurance program which includes a $ 750.00 annual deductible and which shall be subject <br />to a 70/30 co-insurance payment up to a limit of $ 1,5000.00 per year. The Preferred <br />Provider Organization (PPO) shall be subject to a 90/10 co-insurance payment per year up <br />to a limit of $ 1,500.00 per year.
