Bill No. 64-9'1 <br />Page 6 <br />South Bend Police Department who desire single health insurance coverage for the self- <br />funded insurance program of the City of South Bend shall be twenty dollars ($ 20.00) per <br />month in plan years 1998, 1999, 2000 and 2001. The contribution by sworn members <br />and officers of the South Bend Police Department who desire dependent health insurance <br />coverage for the self-funded insurance program of the City of South Bend shall be fifty <br />dollars ($ 50.00) per month in plan years 1998, 1999, 2000 and 2001. The City shall <br />continue to contribute five hundred dollars ($ 500.00) to each qualifying sworn member's <br />and officer's individual flexible spending account, each plan year. <br />N. Wellness and Fitness Prog~m: In the event the City of South Bend establishes <br />a Wellness and Fitness Program by September 15, 2000, a qualifying sworn member or <br />officer electing to participate and successfully participating shall be entitled to a reduction of <br />ten percent (10 %) of his or her monthly premium if participating in the City's <br />comprehensive major medical insurance program. A sworn member or officer who is not <br />participating in the City's comprehensive major medical insurance program who <br />successfully participates in said Wellness and Fitness shall be added to the payment for <br />election to leave the City's Insurance Program of fifty dollars ($ 50.00) per month. All <br />sworn members and officers hired after September 15, 2000, shall be required to <br />participate in the City's Wellness and Fitness Program, if established, as a condition of <br />employment. <br />O. Personal Davs. Each sworn member and officer with more than one (1) full <br />year of service shall receive on January 1st of each year, seven ('~ days of personal time for <br />use in that year. No sworn member shall receive personal time during his or her first full <br />year of service. Upon the first anniversary date of swearing in, each sworn member shall <br />receive personal time to use in the remainder of that calendar year in an amount equal to one <br />(1) day for every fifty (50) calendar days remaining in the year plus one (1) additional day <br />for any fraction remaining, not to exceed a maximum of seven ('n days. Personal days are <br />designed to be used on a "use it or lose it basis" subject to further conditions which maybe <br />established by the Board of Public Safety. Each sworn member shall have the opportunity <br />to earn three (3) additional days of personal time through shooting subject to conditions as <br />may be established by the Board of Public Safety. <br />
