<br />PROJECT NO. 109-005 (ESCROW ACCOIaT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Ancon Construction, Inc., 2146 Elkhart Road, Goshen, Indiana 46527, indicating the Contract
<br />amount be increased by $60,027.00 for a new Contract sum, including this Change Order, in the
<br />amount of $4,173,415.27. Upon a motion made by Mr. Inks, seconded by Mr. Murphy and
<br />carried, the Change Order was approved.
<br />DADDY'S —PROJECT NO. 108-076JLIF)
<br />Mr. Gary Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. I on
<br />behalf of Brown & Brown General Contractors., 124 S. Elkhart Street, Wakarusa, Indiana 4657 ),
<br />indicating the Contract amount be increased by $2,245.00 for a new Contract sum, including this
<br />Change Order, in the amount of $51,472.00. Upon a motion made by Mr. Inks, seconded by Mn
<br />Murphy and carried, the Change Order was approved.
<br />Mr. Gilot advised that Mr. Toy Villa has submitted Change Order No. I (Final) on behalf of LL.
<br />Geans Construction Company, 1923 North Home Street, Mishawaka, Indiana 46545, indicating
<br />the contract amount be decreased by $12,785.40 for a new contract sum, including this Change
<br />Order, of $26,111.75. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost of $26,111.75. Upon a motion made by Mr. Inks, seconded by Mr. Murphy
<br />and carried, Change Order No. I (Final) and the Project Completion Affidavit were approved.

<br />Mr. Gilot advised that Mr. Toy Villa has submitted Change Order No. I (Final) on behalf of
<br />McIntyre Homes, Inc., 260 ) North Foundation Drive, South Bend, Indiana 46628, indicating the
<br />contract amount be decreased by $1,680.23 for a new contract sum, including this Change Order,
<br />of P37,502.05. Additionally submitted was the Project Completion Affidavit indicating this new
<br />final cost of $37,502.05. Upon a motion made by Mr. Inks, seconded by Mr. Murphy and
<br />carried, Change Order No. I (Final) and the Project Completion Affidavit were approved.
