REGULAR MEETING OCTOBER 13 2015 335 Board concerning this matter, the Public Hearing was closed. Therefore, upon a motion made by Ms. Maradik, seconded by Mr. Relos and carried, the Massage Establishment License was approved. APPROVE MINUTES OF PREVIOUS MEETING Upon a motion made by Mr. Relos, seconded by Ms. Maradik and carried, the Minutes of the Agenda Review Session and Regular Meetings of the Board held on September 17, 2015 and September 22, 2015, and the Claims Review Meetings held on September 29 and October 6, 2015, were approved. OPFNINTG AND _AWARD OF BIDS — EQUIPMENT PURCHASE FOR NELLO CORPORATION. PHASE IV —PROJECT NO 115-113 (RWDA TIF) This was the date set for receiving and opening of sealed bids for the above referenced project. The Clerk tendered proofs of publication of Notice in the South Bend Tribune and the Tri- County News, which were found to be sufficient. The following bid was opened and publicly read: GLADWIN MACHINERY & SUPPLY COMPANY OF ILLINOIS INC. 636 E. State Parkway Schaumburg, IL 60173 Bid was signed by: William J. Matrinan, Vice President Contractor's Non -Collusion Affidavit, Non -Debarment Affidavit, Certification Regarding Investment with Iran, Employment Eligibility Verification, Non -Discrimination Commitment for Contractors and Certification of Use of United States Steel Products or Foundry Products was submitted. After reviewing those bids, Mr. Michael Carey, Engineering, recommended that the Board award the contract to the sole responsive and responsible bidder, Gladwin Machinery & Supply Company of Illinois, Inc., 636 E. State Parkway, Schaumburg, IL 60173, in the amount of $411,485.00. Mr. Gilot asked if Nello Corporation had signed off on the award. Mr. Carey stated they reviewed the bids with him and signed off on the award and will follow up with a letter stating so. Therefore, Mr. Relos made a motion that the recommendation be accepted and the bid be awarded and the contract approved as outlined above. Ms. Maradik seconded the motion, which carried.
