<br />APPROVE CHANGE ORDER NO 3 — WESTERN AVENUE STREETSCAPE — PROJECT
<br />NO. 114-085A (TIF ENFRASTRUCTL
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 3 on behalf
<br />of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana 46614,
<br />indicating the Contract time be extended by fifteen (15) days with a new completion date of
<br />November 10, 2015 and no change in the Contract sum, remaining in the amount of
<br />$1,830,627.28. Upon a motion made by Mr. Relos, seconded by Ms. Maradik and carried, the
<br />Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 — LINCOLN WAY WEST CORRIDOR
<br />IMPROVEMENTS — PROJECT NO 114-033 . DIVISION B (LOSS RECOVERY FUND
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 on behalf
<br />of Martell Electric, LLC, 4601 Cleveland Road, South Bend, Indiana 46628, indicating the
<br />Contract be increased by $16,280.90 for a new Contract sum, including this Change Order, in the
<br />amount of $217,790.40. Upon a motion made by Ms. Maradik, seconded by Mr. Relos and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — LINCOLN WAY WEST CORRIDOR
<br />IMPROVEMENTS — PROJECT NO 114-033A DIVISION D (LOSS RECOVERY FUND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Rieth-Riley Construction Co., PO Box 1775, South Bend, Indiana 46614, indicating the
<br />Contract be increased by S17,824.45 for a new Contract sum, including this Change Order, in the
<br />amount of $331,742.45. Upon a motion made by Mr. Relos, seconded by Ms. Maradik and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — LINCOLN WAY WEST CORRIDOR
<br />IMPROVEMENTS —PROJECT NO 114-033A DIVISION E (LOSS RECOVERY FUND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Rieth-Riley Construction Co., PO Box 1775, South Bend, Indiana 46614, indicating the
<br />Contract be increased by $38,289.30 for a new Contract sum, including this Change Order, in the
<br />amount of $833,777.30. Upon a motion made by Mr. Relos, seconded by Ms. Maradik and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE. ORDER NO. 17 —DIGESTER NO. 2 UPGRADE AND DIGESTER
<br />GAS CLEANING —PROJECT NO 111-029 (2012 SEWER BOND)
<br />Mr. Gilot advised that Mr. Jacob Klosinski, Environmental Services, has submitted Change
<br />Order No. 17 on behalf of Grand River Construction Inc., P. 0. Box 323, 5025 40c" Avenue,
<br />Hudsonville, Michigan, 49426, indicating the Contract be decreased by S2,494.00 for a new
<br />Contract sum, including this Change Order, in the amount of S5,980.037.00. Upon a motion
<br />made by Mr. Relos, seconded by Ms. Maradik and carried, the Change Order was approved.
<br />APPROVE CHANGE. ORDER N0. 1 —DEMOLITION OF VACANT &ABANDONED
<br />HOUSES. DIVISION D —PROJECT NO 115-047 (LOSS RECOVERY FUND)
<br />Mr. Gilot advised that Mr. Michael Carey, Engineering, has submitted Change Order No. 1 on
<br />behalf of Green Demolition Contractors, Inc., 1427 Dickens Street, Chicago, Illinois 60614,
<br />indicating the Contract be increased by $1,325.00 for a new Contract sum, including this Change
<br />Order, in the amount of $470,825.00. Upon a motion made by Ms. Maradik, seconded by Mr.
<br />Relos and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO 2 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— REMOVAL AND DISPOSAL OF PCB CONTAMINATED SOIL AT IVY TOWER
<br />PROJECT NO. 1 15-004 (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering has submitted Change Order No. 1 (Final) on
<br />behalf of RCRA, Inc., dba Earth Services, 10903 Prestwick Drive, Benton, Illinois 62812,
<br />indicating the contract amount be decreased by $600.00 for a new contract stun, including this
<br />Change Order, of $92,300.00. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $92,300.00. Upon a motion made by Mr. Relos, seconded by
<br />Ms. Maradik and carried, Change Order No. 2 (Final) and the Project Completion Affidavit were
<br />approved.
