<br />Mr. Gilot advised that Ms. Thia Vawter, Engineering, has submitted Change Order No. 5 on
<br />behalf of Interstate Environmental Services, Inc., 337 West 806 North US Route 6, Valparaiso,
<br />Indiana 46385, indicating the Contract completion date be increased by thirty (30) days (18
<br />working days) for a. new Contract completion date of July 3, 2015. Upon a motion made by Mr.
<br />Rclos, seconded by Ms. Maradik and carried, the Change Order Ams approved.
<br />APPROVE CHANGE ORDER NO. 1 — CATALYST ONE & TWO BUILDING FACADE
<br />ACM PANEL WALLS — PROJECT NO 115-006 (AEDA TIF)
<br />Mr. Gilot advised that Mr. Michael Carey, Engineering, has submitted Change Order No. I on
<br />behalf of Shaffner Heaney Associates, Inc., 2508 South Main Street, South Bend, Indiana 46614,
<br />indicating the Contract completion date be increased by forty-nine (49) days at no additional
<br />cost. Upon a motion made by Mr. Relos, seconded by Ms. Maradik and carried, the Change
<br />Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 — IGNITION PARK INFRASTRUCTURE PHASE 1C
<br />(DIVISION A) —PROJECT NO 114-063A (RWDA TIT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 on behalf
<br />of Walsh & Kelly Inc., 24358 St. Rd. 23, South Bend, Indiana 46614, indicating the Contract
<br />amount be increased by $94,500.00 for a revised contract amount of $3,750,068.51. Upon a
<br />motion made by Mr, Relos, seconded by Ms. Maradik and carried, the Change Order was
<br />approved.
<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— IGNITION PARK SOUTH DEMOLITIONS PHASE VI — PROJECT NO. 115-034(RIVER
<br />WEST DEVELOPMENT AREA T1F)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Pelley Excavating, 1880 Progressive Drive, Niles, Michigan 49120 indicating the
<br />contract amount be decreased by $5,000.00 for a new contract sum, including this Change Order,
<br />of $27,807.00. Additionally submitted was the Project Completion Affidavit indicating this new
<br />final cost of $27,807.00. Upon a motion made by Mr. Relos, seconded by Ms. Maradik and
<br />carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — DECONSTRUCTION OF VACANT &
<br />ABANDONED HOUSES — PROJECT NO 114-029 (LOSS RECOVERY FUND)
<br />Mr. Gilot advised that Ms. Thin. Vawter, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Jackson Services, 1674 N 750 West, Rochester, Indiana 46975, for the
<br />above referenced project, indicating a final cost of $244,000.00. Upon a motion made by Ms.
<br />Maradik, seconded by Mr. Relos and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — CENTURY CENTER ELEVATOR
<br />REPLACEMENT —PROJECT NO 114-021 (HOTEL/MOTEL TAX)
<br />Mr. Gilot advised that Ms. Thia Vawter, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Gibson Lewis, LLC, 1001 West 11" Street, Mishawaka, Indiana 46544,
<br />for the above referenced project, indicating a final cost of $157,600.00. 1,pon a motion made by
<br />Mr. Relos, seconded by Ms. Maradik and carried, the Project Completion Affidavit was
<br />approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE
<br />SHEET — WASTEWATER TREATMENT PLANT SECONDARY TREATMENT
<br />IMPROVEMENTS — PROJECT NO 114-053 (2012 SEWER BOND WASTEWATER
<br />CAPITAL IMPROVEMENTS)
<br />In a memorandum to the Board, Mr. Jacob Klosinski, Environmental Services, requested
<br />permission to advertise for the receipt of bids for the above referenced project. Also presented at
<br />this time for approval and execution was the Title Sheet. Upon a motion made by Mr. Relos,
<br />seconded by Ms. Maradik and carried, the above request to advertise was approved, and the Title
<br />Sheet was approved and signed.
