Exhibit' D <br />Exhibit D: Supplement to Petition for Tax Abatement <br />Proposed Project: The South Bend Clinic, Portage <br />In an effort to better serve its current patient population and be more easily <br />accessible to new patients and population growth, the Clinic has embarked upon <br />expansion at each of four current campuses (downtown, Ironwood at Inwood, <br />Granger, and New Carlisle), and relative to this Petition, acquisition and <br />development of a new campus in a geographic region of the City that the Clinic <br />has not historically served. <br />Subsequent to market analysis and assessment, the Clinic identified the <br />northwest portion of the City as a medically underserved area, one which the <br />Clinic had a material number of patients who accessed its services but drove to <br />its other facilities for care, and one which is undergoing and projected to continue <br />with significant residential and commercial property development and population <br />growth . <br />With expansion of the Clinic's services at its downtown campus and growth of its <br />physician population and capacity to serve at that location, it was determined that <br />two current satellite offices of primary care physicians also in the downtown area <br />could be relocated to better serve their patient base, and better address <br />demographic growth patterns in the region. These two current satellite offices <br />are located in property the Clinic leases from independent third parties, and with <br />the proposed project, will be relocated to a new facility owned and developed by <br />the Clinic through one of its wholly owned real estate entities. <br />The Clinic has and will continue to be afor-profit, tax paying entity. While <br />temporary tax abatement is being requested, the incremental effect of the Clinic's <br />relocation of current offices, and expansion of facilities will result in an <br />incremental tax base for the City. <br />The land on which the development is proposed is located on an approximate <br />13-acre parcel on the southeast corner of Portage and Brick roads. Acquired by <br />the Clinic in late 2005, the Clinic sought and was successful in its initiative to <br />have the land annexed to the City of South Bend. Concurrent with property <br />development, the Clinic will enter into a 50:50 shared venture with the City to <br />share the cost~of utility (sewer and water) extensions north on Portage Ave to the <br />Clinic's property. <br />With expansion on the northwest side of town, the Clinic's satellite offices current <br />and new satellite offices provide primary care medicine and services to the area's <br />population in both suburban and urban center city areas. The satellite suburban <br />locations serve as cost effective sources of primary care, while also acting as <br />referral sources to the Clinic's main hub at its downtown campus.
