CITY OF SOUTH BEND PETITION FOR <br />REAL PROPERTY TAX ABATEMENT CONSIDERATION <br />The undersigned owner(s) of real property, located within the City of South Bend, hereby <br />petition the Common Council of the City of South Bend for real property tax abatement <br />consideration and pursuant to I.C. 6-1.1-12 1-1, et sec ., and South Bend Municipal Code Section <br />2-76, et sec ., for this petition states the following: <br />Describe the proposed redevelopment or rehabilitation project, including information <br />about physical improvements to be made, the proposed use of the improvements, and a <br />general statement as to the importance of the project to your business. <br />The proposed project will convert a current vacant, undeveloped lot into a <br />campus for primary medical care services. The western portion of the land will <br />be developed with a 1.5 story 13 874 gross square foot medical office that will <br />offer the services of up to eight physicians specializing in family medicine and <br />internal medicine. Parking space will consist of 102 paved surface parking <br />spaces. Primary campus access will be from Brick Road. Comprehensive <br />landscaping will be undertaken so as to present a community based, residential <br />medical office complementing area conditions, while the balance of the property <br />which will be undeveloped and available for future use will be seeded with natural <br />wildflowers and grasses. (See additional information on Exhibit <br />D.) <br />Land Size 12.72 acres +/- acres; Building Size 13,874 BGSF <br />2. Describe the overall nature of the business and of the operations occurring at the Property <br />(Please attach additional sheet if more space is needed): <br />The medical office will offer the services of up to eight physicians specializing in <br />family medicine and internal medicine. Supporting the physicians will be a <br />complement of on-site diagnostic services including plain film radiography, <br />screening mammography and basic laboratory services. The physicians will <br />serve a patient population from obstetric and newborn care through children, <br />adolescents adults and the elderly. On site procedural services will include <br />minor office surgery and care of acute inluN, flexible sigmoidoscopy, primary <br />dermatologic services, routine fracture care, and complete physicals and <br />preventative medicine services. (See additional information on Exhibit D.) <br />3. Estimate the dollar value of the project (excluding land): $2,943,000 <br />4. The current assessed valuation of the real property before rehabilitation, redevelopment, <br />economic revitalization, or improvement: $26,100 <br />
