<br />this new final cost of $323,040.00. Upon a motion made by Mr. Relos, seconded by Ms. Roos
<br />and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 (Final) on
<br />behalf of R & R Excavating, 705 South Bciger Street, Mishawaka, Indiana 46546 indicating the
<br />contract amount be decreased by $27,041.80 for a new contract sum, including this Change
<br />Order, of $421,058.20. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost of $421,058.20. Upon a motion made by Ms. Roos, seconded by Mr. Relos
<br />and carried, Change Order No. 2 (Final) and the Project Completion Affidavit were approved.

<br />Mr. Gilot advised that Mr, Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of R & R Excavating, 705 South Reiger Street, Mishawaka, Indiana 46546,
<br />for the above referenced project, indicating a final cost of $200,370.00. Upon a motion made by
<br />Ms. Roos, seconded by Mr. Henthom and carried, the Project Completion Affidavit was
<br />approved.

<br />for the above referenced project, indicating a final cost of $252,750.00. Upon a motion made by
<br />Ms. Roos, seconded by Mr. Relos and carried, the Project Completion Affidavit was approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Office Interiors/Workplace Environments, 1415 University Drive, Court,
<br />Granger, Indiana 46530, for the above referenced project, indicating a final cost of $134,752.00.
<br />Upon a motion made by Ms. Roos, seconded by Mr. Relos and carried, the Project Completion
<br />Affidavit was approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of The Blakley Corporation/F. E. Gates Division, 8060 East 88 Street,
<br />Indianapolis, Indiana 46256, for the above referenced project, indicating a final cost of
<br />$114,239.80. Upon a motion made. by Ms. Roos, seconded by Mr. Relos and carried, the Project
<br />Completion Affidavit was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS TWO (2)

<br />In a memorandum to the Board, Mr. Jeffrey Hudak, Central Services, requested Permission to
<br />advertise for the receipt of bids for the above referenced equipment. Therefore, upon a motion
