<br />APPROVE MINUTES OF PREVIOUS MEETING
<br />Upon a motion made by Mr. Relos, seconded by Mr. Henthom and carried, the Minutes of the
<br />Agenda Review Session, Regular Meeting and Claims Review Meeting of the Board held on
<br />December 4, 2014, December 9, 2014 and December 16, 2014, were approved.
<br />ELECTION OF 2015 BOARD OF WORKS PRESIDENT
<br />Upon a motion by Mr. Relos, seconded by Mr. Henthom and carried, Mr. Gary Gilot was elected
<br />President of the Board of Works for 2015. Mr. Gilot recused himself from the vote.
<br />OPENING AND AWARD OF QUOTATIONS — ENGINEER_ ING SURVEY EQUIPMENT
<br />PURCHASE (MAJOR MOVES)
<br />Mr. Gilot advised that this was the date set for the receiving and opening of sealed Quotations for
<br />the above referenced project. The following Quotations were opened and read:

<br />Quotation was submitted by Mr. Russ Ciesiolka

<br />Commitment for Contractors and Certification of Use of United States Steel Products or
<br />Foundry Products was submitted.
<br />QUOTATION: $31,990.00

<br />Quotation was submitted by Mr. Todd Jamieson
<br />Contractor's Non - Collusion Affidavit, Non - Debarment Affidavit, Certification Regarding
<br />Investment with Iran, Employment Eligibility Verification, Non - Discrimination
<br />Commitment for Contractors and Certification of Use of United States Steel Products or
<br />Foundry Products was submitted.
<br />Indiana Local Business Preference Claim was submitted, but not eligible.
<br />QUOTATION: $59,405.70
<br />Upon a motion made by Mr. Henthom, seconded by Mr. Relos and carried, the above Quotations
<br />were referred to Engineering for review and recommendation. After review, Mr. Michael Carey,
<br />Engineering, recommended that the quotation be awarded to the lowest responsive and
<br />responsible bidder, Positioning Solutions Company in the amount of $31,990.00. Upon a motion
<br />by Mr. Henthom, seconded by Mr. Relos and carried, the award was approved as outlined above.
<br />OPENING OF PROPOSALS — PRE-QUALIFICATION FOR 2015 SEWER INSURANCE
<br />PROGRAM LISTING
<br />Mr. Gilot advised that this was the date set for the receiving and opening of sealed Proposals for
<br />the above referenced program. Ms. Linda Martin, Clerk of the Board, stated the Board received
<br />no proposals for this request.
<br />AWARD BID — ONE 1 OR MORE 2014 OR NEWER JET RODDER/VACUUM MACHINE
<br />WITH CAB AND CHASSIS (2014 SEWER DEPARTMENT CAPITAL]
<br />Mr. Matt Chlebowski, Central Services, advised the Board that on October 14, 2014, bids were
<br />received and opened for the above referenced equipment. After reviewing those bids, Mr.
<br />Chlebowski recommended that the Board award the contract to the most responsive and
<br />responsible bidder Jack Doheny Companies, Inc., 777 Doherty Drive, Northville, Michigan
<br />48167, in the amount of $283,995.00. In his letter to the Board, Mr. Chlebowski noted the two
<br />lowest bids were rejected as non - responsive because they did not meet the specifications.
<br />Therefore, Mr. Henthom made a motion that the recommendation be accepted and the bid be
<br />awarded as outlined above. Mr. Relos seconded the motion, which carried.
