<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 on behalf
<br />of Pelley Excavating, 1990 Progressive Drive, Niles, Michigan 49120, indicating the contract
<br />amount be decreased by $2,776.00 for a modified contract sum, including this Change Order, in
<br />the amount of $654,625.00. Upon a motion made by Ms. Roos, seconded by Mr. Henthorn and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 7 — DIGESTER NO. 2 UPGRADE AND DIGESTER GAS
<br />CLEANING — PROJECT NO. 111 -029 (WASTEWATER O &M, 2011 SEWER BOND
<br />Mr. Gilot advised that Mr. Jacob Klosinski, Division of Environmental Services, has submitted
<br />Change Order No. 7 on behalf of Grand River Construction, Inc., PO Box 323, 5025 40
<br />Avenue, Hudsonville, Michigan, 49426, indicating the Contract amount be increased by
<br />$11,064.00 for a new Contract sum, including this Change Order, in the amount of
<br />$5,767,855.00. Upon a motion made by Ms, Roos, seconded by Mr. Relos and carried, the
<br />Change Order was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — PARKING GARAGE SIGNAGE —
<br />PROJECT NO. 113 -016 (PARKING GARAGE FUND
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of U.S. Signcrafters, 216 Lincolnway East, Osceola, Indiana 46561, for the
<br />above referenced project, indicating a final cost of $50,000.00. Upon a motion made by Ms.
<br />Roos, seconded by Mr. Relos and carried, the Project Completion Affidavit was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE
<br />SHEET — TWO -WAY CONVERSION OF LAFAYETTE BOULEVARD AND WILLIAM
<br />STREET —PROJECT NO. 112 -065 MAJOR MOVES
<br />In a memorandum to the Board, Mr. Roger Nawrot, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Ms. Roos, seconded by Mr.
<br />Relos and carried, the above request to advertise was approved, and the Title Sheet was approved
<br />and signed.

<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Ms. Roos, seconded by Mr.
<br />Relos and carved, the above request to advertise was approved, and the Title Sheet was approved
<br />and signed.

<br />In a memorandum to the Board, Ms. Toni Laisure, Central Services, requested permission to
<br />advertise for the receipt of bids for the above referenced supplies. Therefore, upon a motion
<br />made by Ms. Roos, seconded by Mr. Henthorn and carried, the above request was approved.
