above, shall receive additional compensation in recognition of cumulative service on the South Bend <br />Police Department. Such Longevity Pay shall be paid in 2002, 2003, 2004, 2005 and shall not be <br />attributed to the annual salary base for pension purposes. Said pay shall be paid with regular payroll <br />amounts throughout the designated four year period and shall be as follows: <br />Cumulative Years of Service Commencement Date Amount <br />10-14 years Beginning in the 11"' year $415 <br />15-19 years Beginning in the 16`'' year $575 <br />20+ years Beginning in the 21S` year $1,050 <br />K. Tuition Reimbursement: Qualifying officers and sworn members of the South Bend <br />Police Department participating in the tuition reimbursement program shall be entitled to the <br />following maximum dollar amounts for course tuition <br />Course Grade Maximum Amount Reimbursed by City <br />A 100% of tuition costs only up to a maximum of $1,500 <br />B 50% of tuition costs only up to a maximum of $1,000 <br />Tuition reimbursement shall be on a "first come, first serve" basis. All governing regulations <br />must be complied with by qualifying officers and sworn members in order to be paid any tuition <br />reimbursement. <br />L. Operation Safe Neighborhood Reimbursement: Qualifying officers and sworn members <br />of the South Bend Police Department who establish legal residence within designated areas of the <br />City of South Bend limits and reside on a continuous permanent basis within such designated areas <br />with the City for the period of time he or she is seeking reimbursement shall be entitled to One <br />Thousand Five Hundred and 00/100 Dollars ($1,500.00) of his or her annual mortgage or rental <br />amount. Qualifying officers and sworn members shall be entitled to be reimbursed by the City on <br />a quarterly basis. All governing regulations must be complied with in order to qualify for said <br />reimbursement. <br />M. Life Insurance: The City of South Bend shall continue to provide life insurance <br />coverage on all officers and sworn members of Twenty Thousand and 00/100 Dollars ($20,000.00). <br />N. Health Insurance: For calendar year 2002 only, the City shall maintain anon-Preferred <br />Provider Organization (PPO) medical insurance program which includes a Seven Hundred Fifty and <br />00/100 Dollars ($750.00) annual deductible and which shall be subject to a 70/30 co-insurance <br />payment up to a limit of One Thousand Five Hundred and 00/100 Dollars ($1,500.00) per year. For <br />calendar year 2002 only, the PPO includes a Two Hundred Fifty and 00/100 Dollars ($250.00) <br />annual deductible and which shall be subject to a 90/10 co-insurance payment per year up to a limit <br />of One Thousand Two Hundred Fifty and 00/100 Dollars ($1,250.00) per year. The contribution by <br />officers and sworn members of the South Bend Police Department who desire single health <br />insurance coverage for the self-funded insurance program of the City of South Bend shall be Twenty <br />and 00/100 Dollars ($20.00) per month and for retirees shall be Seventy Eight and 40/10 Dollars <br />($78.40) per month in plan year 2002 only. The contribution by officers and sworn members of the <br />South Bend Police Department who desire dependent health insurance coverage for the self-funded <br />Page 4 of 6 <br />
