<br />City with options to reduce the cost of the bids, if they wanted to call him and discuss. Upon a
<br />motion made by Mr. Gilot, seconded by Mr. Relos and carried, the above bids were rejected.
<br />APPROVE CHANGE ORDER NO. 1 — DIAMOND AVENUE STORM SEWER
<br />SEPARATION PHASE 3 — PROJECT NO 109-033C (2012 SEWER BOND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Woodruff & Sons, Inc., PO Box 450, Michigan City, Indiana 46361, indicating the contract
<br />amount be increased by $23,220.96 for a modified contract sum, including this Change Order, in
<br />the amount of $2,563,209.84. Upon a motion made by Mr. Relos, seconded by Mr. Neal and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 — EAST BANK SEWER SEPARATION PHASE 4 —
<br />PROJECT NO. 112-041 (2012 SEWER BOND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 on behalf
<br />of Selge Construction Co., Inc., 2833 South I I'h Street, Niles, Michigan 49120, indicating the
<br />contract amount be increased by $96,957.00 for a modified contract sum, including this Change
<br />Order, in the amount of $2,737,322.26. Upon a motion made by Mr. Relos, seconded by Mr.
<br />Neal and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — PINHOOK WATER TREATMENT PLANT
<br />EFFICIENCY IMPROVEMENTS —PROJECT NO 112 062
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of L.D. Docsa Associates, 1605 King Highway, Kalamazoo, Michigan 49001, indicating the
<br />contract amount be increased by $17,753.40 for a modified contract sum, including this Change
<br />Order, in the amount of $3,278,179.40. Upon a motion made by Mr. Relos, seconded by Mr.
<br />Neal and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— WILLIAM STREET STREETSCAPE (WASHINGTON STREET TO LASALLE AVENUE)
<br />—PROJECT NO 113-003 (W WCDA TIE)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Rieth Riley Construction, PO Box 1775, South Bend, Indiana 46614, indicating the
<br />contract amount be decreased by $1,194.45 for a modified contract sum, including this Change
<br />Order, of $121,551.65. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost of $121,551.65. Mr. Relos noted the account number on the agenda is not
<br />correct for the WWCDA TIF funding. Upon a motion made by Mr. Relos, seconded by Mr. Neal
<br />and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved
<br />subject to confirmation of the account associated with West Washington TIF.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — CENTURY CENTER LIGHTING
<br />RENOVATIONS — PROJECT NO 112-033 (HOTEL/MOTEL AND CENTURY CENTER
<br />CAPITAL)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Pemberton Davis Electric, Inc., 916 East McKinley Avenue, Mishawaka,
<br />Indiana 46545, for the above referenced project, indicating a final cost of $636,443.00. Upon a
<br />motion made by Mr. Relos, seconded by Mr. Neal and carried, the Project Completion Affidavit
<br />was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — SKYWAY PLAZA BUILDING
<br />WINDOW REPLACEMENT —PROJECT NO 112-043 (MEDICAL TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Gibson -Lewis, LLC, 1001 W. 11 "h Street, Mishawaka, Indiana 46544, for
<br />the above referenced project, indicating a final cost of $303,750,00. Upon a motion made by Mr.
<br />Relos, seconded by Mr. Neal and carried, the Project Completion Affidavit was approved.
<br />APPROVE CERTIFICATE OF SUBSTANTIAL COMPLETION — DEMOLITION AND
<br />RECONSTRUCTION OF FIRE STATION NO 5 — PROJECT NO 111 015 (EMSCIP)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Certificate of Substantial
<br />Completion on behalf of Majority Builders, Inc., 62900 U.S. 31 South, South Bend, Indiana
