A person who has been issued a Handicapped Parking Permit by the City bearing the Permit Number, Permit Holder's Name, signature of the'au- thorizing city official(s), and the official international wheel- chair symbol or a reasonable facsimile thereof, shall be entitled to all of the parking privileges authorized by this article. SEC. 20 -111. Application and Permit Process (a) Any person, for or on behalf of any mobility handicapped person who does not have DisabIled Veteran Registration Plate(s) or Special Registration Plate(s) may file an application for a South Bend Handicapped Parking Permit with the Board of Public Works. (b) Said application shall be provided by the City Con- troller's office and shall "request the following: (1) The legal name of the mobility handicapped person. (2) The present address and.telephone number of the mobility handicapped person. (3) Written certification from a licensed physician stating that (a) the mobility handicapped person has permanently lost the use of a.leg or both legs or an arm or both arms or any combination thereof; or (b) the mobility handicapped person presently suffers from a disability resulting in a physical con i- tion that precludes him from being able to walk more than two city blocks and that this condition shall last for the duration o the present calendar year. (c) Within.thirty (30) days of filing a completed appli- cation form with the Board of Public Works, the Board shall in- struct the City Controller's Office to issue a South Bend handi.- capped Parking Permit to the applicant if it is found that the material facts of the application are true. (d) Upon the issuance of such permit, by the City Controller's office, the permit shall be displayed in the front windshield of the driver's side by the permit holder. (e) Any permit issued pursuant to this Article shall expire on December 31st of the vear issued. SEC. 20 -112. Designation of Handicapped Parking Spaces (a) Any person on behalf of any mobility handicapped person or authorized group organization representing the handicapped may request the Board of Public Works to designate a space or spaces for Handicapped Parking.
