<br />Mr. Gilot advised that Mr. John Wiltrout, Water Works, has submitted Change Order No. 2 on
<br />behalf of Slatile Roofing and Restoration, Inc., 1703 South Ironwood Drive, South Bend, Indiana
<br />46613, indicating the contract amount be increased by $6,880.00 for a modified contract sum,
<br />including this Change Order, in the amount of $41,188.00. Ms. Greene stated she has reviewed
<br />this Change Order and has confirmed the increase is due to unforeseen circumstances and is
<br />therefore, not considered an increase in the scope of the project per Indiana Statute. Upon a
<br />motion made by Ms. Roos, seconded by Mr. Relos and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. I — FIRE STATION NO. 5 DEMOLITION AND
<br />RECONSTRUCTION —PROJECT NO. 111-015 EMS CAPITAL
<br />Mr. Gilot advised that Ms. Thia Vawter, Engineering, has submitted Change Order No. 1 on
<br />behalf of Majority Builders, Inc., 62900 US 31 South, South Bend, Indiana 46614, indicating the
<br />contract amount be decreased by $25,032.00 for a modified contract sum, including this Change
<br />Order, in the amount of $1,116,071.00. Upon a motion made by Ms. Roos, seconded by Mr.
<br />Mecham and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 16 — TRAFFIC SIGNAL MODERNIZATION — PROJECT
<br />NO. 108-065 (FHWA)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 16 on behalf
<br />of Michiana Contracting, Inc., 7843 Lilac Road, P.O. Box 929, Plymouth, Indiana 46563,
<br />indicating the contract amount be decreased by $2,887.41 for a modified contract sum, including
<br />this Change Order, in the amount of $1,281,971.51. Upon a motion made by Mr. Relos,
<br />seconded by Mr. Mecham and carried, the Change Order was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — ONE 1 OR
<br />MORE 2013 OR NEWER JET RODDER/VACUUM MACHINE WITH CAB AND CHASSIS
<br />(2013 SEWER DEPARTMENT CAPITAL)
<br />In a memorandum to the Board, Mr. Matthew Chlebowski, Central Services, requested
<br />permission to advertise for the receipt of bids for the above referenced equipment. Therefore,
<br />upon a motion made by Ms. Roos, seconded by Mr. Relos and carried, the above request was
<br />approved. Mr. Gilot suggested that Mr. Paul South, Director of Streets, talk with the Director of
<br />Public Works on purchasing this truck versus leasing one at 1.1 % interest. He noted at that low
<br />rate, leasing may be a better solution.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR ,THE RECEIPT OF BIDS --- STREET
<br />SWEEPING DEBRIS DISPOSAL — PROJECT NO. 113-020 SEWERS
<br />In a memorandum to the Board, Mr. Paul South, Street Department, requested permission to
<br />advertise for the receipt of bids for the above referenced service. Therefore, upon a motion made
<br />by Ms. Roos, seconded by Mr. Relos and carried, the above request was approved.
<br />ADOPT RESOLUTION NO. 37-2013 — A RESOLUTION OF THE SOUTH BEND BOARD
<br />OF PUBLIC WORKS APPROVING THE TRADE-IN OF CITY -OWNED PROPERTY ON
<br />PROPERTY OF SIMILAR NATURE
<br />Upon a motion made by Ms. Roos, seconded by Mr. Relos and carried, the following Resolution
<br />was adopted by the Board of Public Works:

<br />A RESOLUTION OF THE SOUTH BEND BOARD OF PUBLIC WORKS
<br />APPROVING THE TRADE-IN OF CITY -OWNED PROPERTY
<br />ON PROPERTY OF SIMILAR NATURE
<br />WHEREAS, it has been determined by the Board of Public Works of the City of South Bend that
<br />the following described property is unfit for the purpose for which it was intended and should be traded -in
<br />for property of a similar nature:
<br />ONE (1) 2006 CHEVROLET IMPALA VIN # 2G1WT58K569300127
<br />WHEREAS, I.C. 36-1-11-9 authorizes the Board of Public Works to trade in or exchange
<br />City -owned property for partial or full reduction in the purchase price of property of similar nature.
<br />NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF PUBLIC WORKS OF THE
<br />CITY OF SOUTH BEND, AS FOLLOWS:
<br />I. The following described property shall be traded -in for property of a similar nature and
<br />shall be removed from the inventory of the City of South Bend:
