C� <br />Premium Concrete Services, Inc. <br />712 Richmond Street <br />Elkhart, IN 46516 <br />(574) 264-0196 <br />SECTION II PLAN AND EQUIPMENT QUESTIONAIRE <br />1) Project: MADISON STREET TRAIL CONNECTOR - PROJECT # 125-031 -Schedule: <br />After given the notice to proceed, Premium Concrete Services, Inc. will contact Indiana 811 to <br />locate all utility lines within construction limits. Traffic control will be completed per plans. Layout <br />and staking of sites will be next. Then removals, excavation, grading, and subgrade with a 3-man <br />crew. Underground work will commence with a 4-man crew as soon as hard surface has been <br />removed. After excavation, underground and grading phases are complete, electric & lighting <br />work will begin. Forming and pouring of concrete will be completed with a 5-man crew. Brick work <br />will begin after curb has been cured for a few days. Topsoil, tree planting & sodding will be <br />completed by a subcontractor. Milling will be completed by a subcontractor. HMA surface <br />completed with 5-man crew. Finally, pavement markings by a subcontractor will complete the <br />project. Subs will be utilized on this project to complete traffic control, landscaping, electrical, <br />lighting, trucking, milling & pavement markings. New Carlisle, IN 46552 — Painting <br />g) Slusser's Green Thumb, Inc. — 1610 Genesis Dr LaPorte, IN 46350 - Landscaping <br />h) Traffic Control Specialists —1810 W Pacific Ave Knox, IN 46534 —Traffic Maintenance & Signs <br />Premium Concrete Services, Inc. Does not require bonding for its subcontractors <br />3) Will supply list of subcontractors soon after award of project. <br />4) Equipment use for project: Skid Steers, Mini Excavators, Excavators, Compactors, Rollers, Walk - <br />behind Saws, Cut-off Saws, Friction Screed, Dump Trucks, Pavers. Current plan is to used owned <br />equipment, however depending on workload some rental equipment may be necessary. <br />5) Yes, Premium Concrete Services, Inc. has quotes for all materials in proposal. <br />
