I acknowledge that I have read and understand this page. Initials:SW <br />their attic is insulated properly, and that the insulation is not restricting intake ventilation from eave vents. It is also the <br />responsibility of the homeowner to make sure that all exhaust vents (i.e. dryers, range hoods, bathroom vents, etc.) are <br />properly ventilating to the exterior of the home. A&M is not responsible for moisture buildup, condensation, installation <br />of soffit vents, mold or mildew growth in your home relating to improper or insufficient air intake and insulation. If it is <br />discovered that cathedral areas of your roof do not contain baffles which facilitate proper air flow A&M will need <br />to install at $4.00 per baffle. If boards need to be removed and replaced to install baffles, the charge will be $3.00 <br />per foot. Plywood will be removed to determine if baffles are present. If the existing plywood is found to be in <br />good condition, those sheets will be reinstalled. The price to REMOVE AND REINSTALL Plywood is $35 per sheet. <br />If the existing plywood is found to be rotten or cracked and needs to be replaced with NEW plywood, the <br />plywood price on the first page of the contract will be charged. <br />BROAN VENTSWhen applicable, A&M will install new broan vent(s) into existing locations or cut in additional broan <br />vents and connect to existing bath fans with a flex hose. This will allow for the warm, humid air to escape out the roof <br />instead of into the attic space. A&M is NOT responsible for broan vents or bath fans that come dislodged after the initial <br />installation, nor re-connecting broan vents at a future date. It’s the responsibility of the customer to ensure the vents <br />are properly secured and fastened to exhaust pipes. <br />ICE DAMMINGIce damming occurs when gutters and downspouts freeze. Melting snow has nowhere to go, so water <br />gets between shingles, which can cause leaks. This is not a problem related to the roofing workmanship. This problem <br />is out of our control. A&M will not be responsible for leaks that occur because of ice damming. <br />SEEPAGE AND WATER IN BASEMENTSA&M is not responsible for any seepage or water leakage in basements, which <br />may occur in the absence of gutters and downspouts. If not written and specified on the contract where the <br />downspouts should go, they will be installed back at the original location. <br />ANTENNAS,please notify the office before work begins if you want your antenna removed. We will remove and/or <br />dispose of antennas. We will not install new antennas. A&M will not be responsible for antenna reception. <br />SATELLITE DISHESA&M will lift your satellite dish and will reset it where it is currently located and will ensure proper <br />flashing of the dish. However, A&M is not responsible for any damage caused to the dish during this process or <br />responsible for the reception of the dish. No exceptions will be made. If you prefer, you may contact your Satellite <br />provider and have them remove the dish prior to the start of the job and reinstall it upon completion of the job. <br />GUTTER REMOVAL AND REINSTALL If A&M is replacing fascia and soffit, but not existing gutters, and wood fascia <br />boards need to be replaced we will charge $75.00 per every 50’ of gutters that need to be removed and reinstalled. This <br />price does not include wood charges that are stated in the contract. <br />GUTTER GUARDS If you have existing gutter guards and A&M needs to remove and replace them to complete your <br />project. A&M cannot be responsible for minor damage due to this process. If you would prefer to remove them yourself <br />prior to the installation, please have this done no later than one week from the date of your contract signing. <br />SPECIFICS PERTAINING TO WINDOW AND DOOR PROJECTS: <br />WINDOW TREATMENTSIt is the responsibility of the homeowner to remove and later reinstall any and all window <br />treatments prior to the start of your job. A&M will not be responsible for shades, blinds, or window treatments of any <br />kind if not removed prior to job start. When installing replacement windows there is the possibility that your existing <br />blinds/window treatments may no longer fit in the opening. M&M cannot be responsible for this. Please see the <br />measure technician for further explanation, if necessary. <br />FURNITURE & APPLIANCESIt is the responsibility of the homeowner to remove all furniture and appliances a <br />minimum of three feet away from all windows and doors to allow access for installation. A&M is not responsible for <br />damage caused to furniture or appliances that are not moved prior to the start of your job. <br />CAULKINGis considered routine maintenance and therefore it is the responsibility of the homeowner. <br />WOOD REPLACEMENTUnless previously agreed upon, any weak or rotten wood which is visible to our crew during the <br />window/door installation will be replaced as necessary. The additional cost for the customer will be as follows: $4.85 per <br />foot for all 1x4, 1x6 and 1x8 replacement and $10.50 per foot for any 2x4, 2x6 and 2x8 replacement. Any wood that is <br />replaced by A&M will not be primed or painted unless otherwise stated.
