z., rCTf1�-T- <br />Will your event have food sales (food vendors, caterers, food trucks, etc.)? ® Yes ❑ No <br />We are working to procure food trucks already permitted by the city. <br />• o If yes, the event coordinator must apply for and receive a St Joseph County Health Department <br />Temporary Event Permit. <br />o Vendor(s) must also apply for and receive a St. Joseph County Health permit. Health Permits must be <br />filed with the county 30 days prior to the proposed event. Each vendor must obtain necessary permits to <br />serve on -site and display these permits at the event. <br />o All applications and guidelines can be found on the St. Joseph County Health Department Food Service <br />website at sichd.orp,/food-service. NOTE: This form was not accessible on website. <br />Please select food types: ❑ Food Vendor ❑ Caterer ® Food Truck ® Other: Carry -in picnic by neighbors <br />If a Food Truck, please list company name(s): <br />We are seeking food trucks who are already permitted in the city, but have not procured any to date. <br />Please describe how food will be cooked and served: <br />Either purchased at food trucks, or provided by neighbors and served buffet style. If food trucks don't work out, we <br />will have volunteer neighbors to prepare meats on grills on -site and potluck sides on tables. In either case, non- <br />alcoholic beverages will be offered in coolers.
