testing. <br />3. Have no more than 3 moving violations in the previous 3 years, or more than <br />one within the last 12 months. No serious violations within the past 3 years. A <br />valid MVR must be reviewed before driver may operate Company equipment. <br />.............. <br />4. Must pass a DOT required substance abuse screen. <br />5. Driver has no DWI, DUI, nor reckless driving convictions in the past 5 years. <br />6. Has had no more than 1 preventable vehicle accident in the past 5 years, no <br />.............. <br />preventable accidents in the past 12 months. <br />7. Has had no accident involving a lawsuit that has not been settled. <br />8. Has had no preventable accident which involved a fatality. <br />9. Has no suspension of driving privileges within the past 5 years, excluding <br />suspension for not paying child support, medical card registration and/or <br />similar offenses. <br />10. Has taken and passed a Written Test and a Road Test. <br />11. Has NOT been assigned a Risk Rating Value of "Questionable" or "High". <br />If a new hire employee does not have a valid CDL license, R. Yoder Construction <br />will assist the employee with the necessary training and testing. Further, R. <br />.............. <br />Yoder Construction will reimburse the employee for the first round of testing. <br />If the employee does not pass the first time, they will be responsible for all fees <br />for additional rounds of testing until they pass. R. Yoder Construction will also <br />reimburse for the employee's CDL Physical Examination. A copy of a valid physical <br />examination must be on file in the Company's office. Failure to receive a valid CDL <br />will result in disciplinary actions being taken. <br />5.9 Expense Reimbursement <br />.............. <br />R. Yoder Construction will reimburse certain expenses incurred on behalf of the <br />.............. <br />Company. Please contact Management for specific details. <br />.............. <br />5.10 Jury Duty <br />.............. <br />If you receive a court order to perform jury duty service or appear for a jury duty <br />service or appear for a jury examination during regular work hours, show the order <br />.............. <br />to HR as soon as possible so adjustments may be arranged. <br />.............. <br />R. Yoder Construction will continue providing current benefits for the term of the <br />jury duty absence. Any qualified vacation and holiday benefits will continue to <br />accrue during jury duty leave. <br />.............. <br />In order to encourage a sense of civic responsibility, R. Yoder Construction will <br />continue the employee's average or normal daily compensation, up to 3 days within <br />a calendar year, while serving for jury duty.
