This proposal represents a legal agreement between <br />JQOL Inc., the designer of record and City Engineer – South Bend (Client). We will provide the Basic <br />Services as described in Project Description below. Terms and Conditions shall be as provided in this <br />proposal. <br />Project Description (Civil) <br />1. Project is located along an 1,800’ section of Mayflower Road in the City of South Bend, Indiana. <br />2. The services rendered will be Surveying the existing section of roadway and designing a new crowned <br />roadway with shoulder for the planned stretch from the county to state line . <br />3. The study will identify the existing issues, note any constraints, and present options with an engineer’s <br />estimate for each option to be reviewed and selected by the city engineering staff. <br />4. Additional design effort will be supplemented as approved by the city. <br />5. Preparation of bid documents related to the construction of a 2-lane collector/commercial roadway <br />within the City of South Bend Indiana including but not limited to the shown boundaries of Mayflower <br />Road. <br />6. Repair and replacement of existing pavement sections within the work area with visual review and <br />documentation. We are assuming that drainage analysis and design will NOT be required and any <br />permitting for additional drainage as well as a drainage report w ill NOT be included as a part of our <br />scope. <br />a. Preparation of design documents including the following: <br />1. Cover sheet, base topographic file, site plan, grading plan, road plan, <br />profiles, and sections, erosion control, details and specs <br />b. Base Bid <br />i. Design roadway from County Line South to Michigan State Line tracing the existing <br />centerline of the drive aisle – highlighted area in green as primary base bid <br />7. Construction Pricing updates at the end of DD and CD phases <br />8. Meetings <br />a. Design Team meetings (as needed during design phase) - virtual <br />i. Meeting Agenda Preparation <br />ii. Meeting Minutes <br />b. Bi-weekly Site Progress meetings during CA phase – onsite <br />c. Pre-Bid meeting <br />i. Lead pre-bid meeting, provide a sign-in sheet, prepare agenda, and follow up with <br />an addendum
