(2) The name and place of business of the applicant. (3) Letter(s)from former employer(s);of a union:or self-employment documentation to show that the applicant has had not less than four(4)years'(or equivalent)experience in the installation,repair and maintenance of residential,commercial or industrial electrical wiring,apparatus or equipment. (4) Payment of a fifty-dollar($50.00) application review fee. (5) Evidence of the applicant's age being at least twenty-one(21)years. (6) An official statement from a law enforcement agency of whether or not the applicant has ever been convicted of a misdemeanor or felony,and if so,the crimes convicted of and the dates and places of such convictions.
