i. <br />Leaving the collection site prior to test completion. <br />ii. <br />Failure to attempt to provide a breath or urine specimen. <br />iii. <br />Failure to provide a sufficient quantity of urine or breath without a without a valid <br />medical explanation (confirmed by a physician). <br />iv. <br />Failure to undergo a medical evaluation when required. <br />v. <br />Failure to cooperate with any part of the testing process. <br />vi. <br />Failure to permit an observed or monitored collection when required. <br />vii. <br />Failure to follow an observer's instructions to raise and lower clothing and turn <br />around. <br />viii. <br />Possessing or wearing a prosthetic or other device to tamper with the testing <br />process. <br />ix. <br />Failure to take second test when required. <br />x. <br />Admitting the adulteration or substitution of a specimen to the collector or MRO. <br />A. <br />MRO verified adulterated/substituted sample. <br />xii. <br />Refusal to sign Step 2 of the alcohol test form. <br />xiii. <br />Failure to appear at a designated collection site within a reasonable time. <br />xiv. <br />Tampering, adulterating, or substituting a breath or urine specimen; and <br />xv. <br />Leaving the scene of an accident without just cause prior to submitting to a test. <br />(An employee may leave the scene of an accident in order to obtain medical <br />assistance for those injured in the accident. Failure to return to the scene will be <br />regarded as a refusal to test.) <br />c. For Pre -Employment Testing, it will be considered a refusal if the applicant: <br />i. <br />Fails to appear for the test; or <br />ii. <br />Leaves the collection site or aborts collection prior to the start of the test. <br />13.
