Page 8 <br />UTILITY COORDINATION <br />Per Indiana 811 Tickets the following utilities reside within the project limits. <br /> <br />VS shall coordinate with utilities in accordance with the following tasks from Indiana Administrative <br />Code (IAC) 105 Article 13: Utility Facility Relocations on Construction Contracts. <br /> <br />• Initial Notice of proposed improvement project <br />• Verification of existing utilities <br />• Work Plan development <br />• Relocation notification <br /> <br />Initial Notice shall contain: <br /> <br />• Geographical limits of project and general description of work to be done. <br />• Anticipated Final Submittal date and Construction date <br />• VS contact information <br /> <br />Verification of Existing Utilities shall comprise of: <br /> <br />• Reviewing the accuracy of the field survey plan as to the location of existing facilities with utility. <br />• When available, acquisition of depth information of underground facilities from utility. <br />• Revisions to plan depicting existing conditions based on direction from utilities. <br /> <br />Work Plan Development shall comprise of: <br /> <br />• Determine conflicts between existing utility facilities and proposed improvements. <br />• Coordinate with utilities to develop a plan to mitigate conflicts. <br />• Utilize final project design for utility use. <br />• Finalize utility work plans. <br /> <br />Relocation Notification shall comprise of: <br /> <br />• Notify utility to implement their relocation work plan. <br /> <br />Coordination with Utilities shall comprise of: <br /> <br />• One (1) Initial Notice per utility. <br />• One (1) round of revisions on verification of Existing Utilities per utility. <br />• One (1) on-site meeting with utilities in conjunction with project’s Preliminary Field Check <br />meeting. <br />• One (1) round of revision to relocation work plans per utility upon finalization of project design <br />elements associated with utility. <br />• General coordination between VS and utilities on status of project and utility information. <br /> <br />Utility Relocations and reimbursement of utility relocations is not anticipated. If necessary, coordination <br />and preparation of relocation plans and utility reimbursement agreement(s) shall be an additional <br />service.
