<br />AWARD QUOTATION — 2013 TRAFFIC PAINT (MVH)
<br />Mr. Paul South, Director of Streets, advised the Board that on April 9, 2013, quotations were
<br />received and opened for the above referenced material. After reviewing those quotations, Mr.
<br />South recommended that the Board award the contract to the lowest responsive and responsible
<br />bidder, Sealmaster Indianapolis, 1010 E. Summer Ave., Indianapolis, Indiana, 46227, in unit
<br />prices bid. Therefore, Mr. Inks made a motion that the recommendation be accepted and the
<br />quotation be awarded as outlined above. Ms. Roos seconded the motion, which carried.
<br />APPROVE CHANGE ORDER NO. 3 — WASTEWATER TREATMENT PLANT
<br />AUTOMATION OF DO CONTROLS FOR AERATION BLOWERS — PROJECT NO. 111 -070
<br />DIVISION OF ENVIRONMENTAL SERVICES CAPITAL
<br />Mr. Gilot advised that Mr. Jacob Klosinski, Environmental Services, has submitted Change
<br />Order No. 3 on behalf of Mason Engineering & Consulting, Inc., 5720 Huguenard Road, Fort
<br />Wayne, Indiana 46818, indicating the contract amount be increased by $53,123.00 for a modified
<br />contract sum, including this Change Order, in the amount of $749,773.60. Upon a motion made
<br />by Mr. Inks, seconded by Ms. Roos and carried, the Change Order was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS -- INFRARED_
<br />PAVEMENT PATCHING 2013 — PROJECT NO. 113 -014 (STREETS/PAVING)
<br />In a memorandum to the Board, Mr. Paul South, Street Department, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Therefore, upon a motion made
<br />by Mr. Inks, seconded by Ms. Roos and carried, the above request was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS
<br />In a memorandum to the Board, Mr. Jeffrey Hudak, Central Services, requested permission to
<br />advertise for the receipt of bids for the following vehicles and equipment:
<br />A. One (1), or More, 2013 or Newer, One Ton Four Wheel Drive Extended Cab Pick
<br />Up Truck
<br />1. Funding: 2013 Traffic Capital Lease Principal and Interest
<br />B. One (1), or More, 2013 or Newer, 18,000 lb Four Wheel Drive Chassis with
<br />Dump Bed
<br />1. Funding: 2013 Park Department Capital
<br />C. One (1), or More, 2013 or Newer Single Axle Dump Truck with Dump Bed
<br />1. Funding: 2013 Code Enforcement Capital
<br />D. One (1), or More, 2012 or Newer Track Drive Stump Grinder
<br />1. Funding: 2013 Park Department Capital Budget
<br />Therefore, upon a motion made by Mr. Inks, seconded by Ms. Roos and carried, the above
<br />request was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE
<br />SHEET — WASTEWATER TREATMENT PLANT DIGESTER NO. 2 UPGRADE AND
<br />DIGESTER GAS CLEANING (WASTEWATER O &M, 2011 SEWER BOND)
<br />In a memorandum to the Board, Mr. Jacob Klosinski, Environmental services, requested
<br />permission to advertise for the receipt of bids for the above referenced project. Also presented at
<br />this time for approval and execution was the Title Sheet. Upon a motion made by Mr. Inks,
<br />seconded by Ms. Roos and carried, the above request to advertise was approved, and the Title
<br />Sheet was approved and signed.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE
<br />SHEET — CSO 18 BACKWATER GATE IMPROVEMENTS — PROJECT NO 112 -060
<br />(REBID) (ENVIRONMENTAL SERVICES)
<br />In a memorandum to the Board, Mr. Rick Smigielski, Environmental Services, requested
<br />permission to advertise for the receipt of bids for the above referenced project. Also presented at
<br />this time for approval and execution was the Title Sheet. Upon a motion made by Mr. Inks,
<br />seconded by Ms. Roos and carried, the above request to advertise was approved, and the Title
<br />Sheet was approved and signed.
