Klosinski, P.E., Assistant City Engineer <br />SUBJECT: Agenda Request: Amendment No. 2 to Professional Services Agreement <br />121-070 Fellows Booster Pump Station <br />(PROJ00000032 / PO-0012147) <br />DATE: July 15, 2024 <br /> <br />The Division of Engineering and Division of Water Works requests the Board of Public Works <br />approval of Amendment No. 2 to a Professional Services Agreement with Donohue & Associates for <br />Construction Administration Services related to Project No. 121-070 Fellows Booster Pump Station. <br /> <br />The full scope of work for Amendment No. 2 and the Construction Administration Services is <br />attached. In summary, the work will include, but not be limited to, shop drawing submittal reviews, <br />periodic on-site inspection, substitution request reviews, change order reviews, and construction <br />observation during start-up and commissioning services. <br /> <br />The funding for the Construction Administration Services for this contract will be funded through the <br />Indiana Finance Authority State Revolving Loan Fund Bond. <br /> <br />There will be no change to the existing purchase order (PO-0012147) since it will be tracked <br />separately through IFA-SRF invoicing for disbursement from the loan proceeds. <br /> <br />Funding for this project was provided through the Indiana Finance Authority State Revolving <br />Loan Fund. <br /> <br />For questions regarding the award recommendation, contact Jacob M.
