a <br />a <br />a <br />The MRO may request from the laboratory and the laboratory will <br />provide quantification of test results. The MRO will then report to Visu- <br />Sewer whether the test is positive or negative and, if positive, the <br />identity of the drug(s) for which there was a positive result. The MRO <br />will not, however, disclose the quantification of test results to Visu- <br />Sewer unless disclosure of the same to the employer, employee or <br />decision-maker is necessary in a lawsuit, grievance or other <br />proceeding initiated by or on behalf of the employee arising from a <br />verified positive drug test. <br />The laboratory will also provide the employer or its agent with a <br />statistical summary of the testing program, which will not include any <br />personal identifying information, as required by any applicable <br />regulation. <br />Designated Collection Sites <br />The facilities listed on APPENDIX F - AUTHORIZED MEDICAL <br />FACILITIES/COLLECTION S/IES or any medical facility with <br />approved collection sites which have the personnel, materials, <br />equipment, facilities and supervision necessary to provide for the <br />collection, security, temporary storage and shipping of urine <br />specimens to an appropriate laboratory for testing: <br />A designated collection site may be any suitable location where a <br />specimen can be collected in compliance with 49 CFR Part 40, <br />including a properly equipped mobile facility. Any such site shall be a <br />location having an enclosure within which private urination can occur, <br />a toilet for completion of urination (unless a single-use collector is used <br />with sufficient capacity to contain the void), and a suitable clean <br />surface for writing. The site must also have a source of water for <br />washing hands, which, if practicable, should be external to the <br />enclosure where urination occurs. <br />Security <br />Any designated collection site shall have in place procedures designed <br />to prevent unauthorized access which could compromise the integrity <br />of the collection process or the specimen. <br />Ghain Of Custody And Gollection Control <br />To the maximum extent possible, collection site personnel shall keep <br />the individual's specimen bottle within sight both before and after the <br />individual has urinated. After the specimen is collected, it shall be <br />properly sealed and labeled. The chain-of-custody block on the Urine <br />Custody and Control Form shall be executed by authorized personnel <br />upon receipt of the specimen. This form shall be used for maintaining <br />control and accountability of each specimen from the point of <br />34
