The employee will not be qualified to work for Visu-Sewer until a professional in substance <br />abuse treatment certifies that the employee has been rehabilitated. ln the event Visu- <br />Sewer does not consider the substance abuse professional chosen by the employee to be <br />acceptable, Visu-Sewer reseryes the right to request a second evaluation by an acceptable <br />treatment professional at Visu-Sewer's expense. The evaluation will be conclusive <br />evidence of the employee's rehabilitation or failure to rehabilitate. The employee may be <br />permitted to return to work iflwhen he/she is able to demonstrate the successful completion <br />of such assessment and/or any recommended treatment. <br />REEMPLOYMENT OPPORTUNITIES <br />ln the event the employee has completed an evaluation, treatment and rehabilitation <br />program, Visu-Sewer may offer the employee the invitation to reapply and interview for the <br />position he or she held at the time of termination. This re-application process must be <br />within 60 days of the employee being released back to work by his or her treatment <br />professional. Visu-Sewer does not guarantee an offer of employment would be granted. <br />The decision to offer employment would be at the judgment of Visu-Sewer based upon the <br />evaluation of the information received either verbally or in written document at the time of <br />application and interview. As a condition of return, the employee will be required to submit, <br />on demand, to drug follow-up for a period of up to 60 months. The number and frequency <br />of such tests is determined by the substance abuse professional at its sole discretion. A <br />positive test during or following the monitoring period will result in immediate termination <br />with no further consideration for future employment. Rehabilitation after a positive test, if <br />permitted at all, will be permitted only one time. <br />prsctPLlNARYAcTtoN, 382.601 (b)4,9. Prospective employees refusing to complete and sign the consent/authorization form, <br />or the chain of custody form at the collection site or to provide the specimen for testing <br />will be denied employment. <br />Prospective employees receiving a positive drug and/or alcohol test result will be <br />denied employment. <br />Any employee refusing to comply with this Policy will be terminated from employment. <br />Any employee refusing to submit to testing requirements will be terminated from <br />employment. <br />Any employee refusing to complete and sign the consenUauthorization form, or the <br />chain of custody form at the collection site or to provide the specimen for testing will be <br />terminated from employment. <br />Any employee who engages in any conduct that constitutes a refusal to submit to a <br />controlled substance or alcohol test required under this Policy will be subject to <br />immed iate termination. <br />a <br />a <br />a <br />a <br />a <br />t2
