a <br />a <br />medical review organization contracted and authorized by Visu-Sewer to conduct <br />the random selection. Separate and distinct random testing pools have been <br />established for non-DOT and DOT employees. Appropriate safeguards have been <br />established to ensure that the identity of individual employees cannot be determined <br />prior to or at the time of their selection. Whenever an employee is randomly <br />selected to be tested, he/she will be notified and instructed to report immediately to <br />the collection site. <br />Periodic <br />Periodically, employees in safety-sensitive positions will be required to submit to an <br />unannounced test for controlled substances. Selection will be determined by Visu- <br />Sewer's management or by the requirements of any government agency or private <br />business customer to which Visu-Sewer has established a contract for work <br />performance (e.9. required testing of Visu-Sewer's employees by a customer for the <br />express purpose of allowing Visu-Sewer's employees onto the job site). Such <br />periodic testing may take place at any time during a calendar year. Employees will <br />have an equal chance of being selected to be tested each and every time the <br />selection for periodic testing is conducted. Appropriate safeguards have been <br />established to ensure that the identity of individual employees cannot be determined <br />prior to or at the time of their selection. Whenever an employee is selected to be <br />tested, he/she will be notified and instructed to report immediately to the collection <br />site. <br />Post-Accident /lnjury <br />As soon as practicable after an accident or injury, each employee directly involved <br />or whose performance contributed to an accident or injury must submit to a drug <br />and/or alcohol test. An accident is defined as: an OSHA recordable incident, acts or <br />omissions resulting in near misses, and accidents involving a personal injury that <br />requires treatment at a medical care facility; incident causing personal injury to <br />another person who requires medical treatment at a medical care facility; and/or <br />property or vehicle damage of $1,000.00 or more. The Department of Labor <br />Occupational Safety and Health Administration (OSHA) defines a recordable <br />incident as a work-related injury or illness that results in death, loss of <br />consciousness, days away from work, restricted work activity or job transfer, or <br />medical treatment beyond first aid. (APPENDIX H - POST ACCIDENT/INJURY <br />INSTRUCTIONS). ln the event, post-accident testing is not administered within 2 <br />hours of the accident, circumstances preventing the tests from being administered <br />must be documented by the employee's supervisor. ln the event, post-accident <br />alcohol testing is not administered within 8 hours of the accident, circumstances <br />preventing the tests from being administered must be documented by the <br />employee's supervisor. ln the event, post-accident controlled substances testing is <br />not administered within 32 hours of the accident, circumstances preventing the tests <br />from being administered must be documented by the employee's supervisor. <br />6
