oxr.wnv. <br />oeMoiirioa ar�cea DBE/`NBE/FSE certified <br />March 25, 2024 <br />Re: Demolition Plan-Drewys Brewery Cleanup <br />South Bend, IN <br />Subject:Alpine Demolition Services, LLC (Alpine) demolition plan of the existing buildings at the above location. <br />1. Existing Conditions: <br />• 2-one story buildings. Slabs and foundations from an old building are still on site. <br />2. Safety and Site Security: <br />• All personnel on site have been trained by Alpine Demolition on safety and OSHA rules regarding demolition. <br />• Proper PPE to be worn by all personnel on site. <br />o Hard hats, safety glasses, highly visible safety vests, steel toe work shoes. <br />o Dust masks or half face respirators in dusty conditions. <br />3. Mobilization/Construction Amass: <br />• Equipment will be delivered via semi low boys and stored on site within the work area for the duration of the <br />specific demolition activities. <br />• All employees to obtain a gate pass for daily entry. <br />• All deliveries will access the jobsite via the East gate. <br />• Access to the work zone to be limited to Construction personnel only. <br />4. TmffcControl <br />o Excavator and skid steer will be kept on site within project limits. Dump truck loading will be handled within project <br />limits. Flagman will be present when trucks are entering and leaving the job site. <br />5. Emergency Response: <br />• Emergency services on this project can be accessed by 911 <br />• Clinic: TBD <br />• All personnel to sign in at beginning of shift. <br />6.
