Page 7 (Exhibit K – (Amendment to Owner-Engineer Agreement) – Attachment 1) entrances. It will also delineate the disturbed and preserved areas and proposed locations of soil stockpiles and borrow areas and other staging elements, as required. 3. Submit Report and SWPPP to the local reviewing agency for 30-day review period. Monitor the approval process and provide customary additional information that may be required to secure permit coverage. 4. Prepare and publish Public Notice. 5. Complete draft application via the IDEM Regulatory ePortal, including upload of proof of publication and local review agency approval forms. Provide completed State Form 56679 Construction Stormwater Signed Certification for Owner signature. 6. Submit final application to IDEM Regulatory ePortal along with $175 IDEM permit fee. Submit State Form 56679 via mail no later than 48 hours prior to start of construction. IDEM will provide Notice of Sufficiency directly to Owner documenting approved permit number. 7. If requested, assist with submission of Notice of Termination form via the IDEM Regulatory ePortal at completion of construction. B. DLZ shall prepare and submit the documentation to apply for permit under the Indiana Department of Homeland Security Division of Fire and Building Safety. The scope shall include completion and submission of the Swimming Pool Application State Form 43038 and required supporting documentation. Monitor approval process and provide any requested typical additional information that may be required to secure permit. C. If required, DLZ will prepare and submit documents for City of South Bend’s Site Plan Review process. 1. Improvement Location Permits and other construction phase permitting are not included and shall be completed by successful bidder via the Building Department. c. The schedule for rendering services is modified as follows: Add to end of A1.05 Schedule: Design and Construction Documents a) Topographic and Boundary Survey (including locates) to be completed within 90 days of notice to proceed. b) Geotechnical Engineering Exploration to be completed within 90 days of notice to proceed. c) 60% Design Phase to be completed within 60 days of survey completion. d) 90% Design Phase to be completed within 60 days of Owner approval of 60% Design deliverable. e) Final Design Phase to be completed within 30 days of Owner approval of 90% Design deliverable.
