<br />INDOT Process for LPA-Consultant Approval <br /> <br />1. Submit an unsigned final draft of the LPA-Consultant Contract with appendices and all <br />supporting documentation provided herein to the INDOT LaPorte District Local Programs <br />Coordinator for approval. <br />a. Include the following: <br />i. Copy of the fee proposal <br />ii. LPA-Consultant Contract <br />iii. Copy of the current INDOT approved overhead rate letter <br />iv. Copy of INDOT Pre-Qualification letter <br />v. LPA-Consultant Contract Review Checklist <br />2. Once INDOT has returned the approved final draft of the LPA-Consultant Contract, we <br />can each execute the contract. <br />3. Submit the executed LPA-Consultant Contract with original signatures to the INDOT <br />LaPorte District Local Programs Coordinator. They will then give final approval and <br />request FMIS authorization for this project. <br />4.
