Letter of Notice for Random Testing <br />Date: (Example: April 82 2016) <br />Re: Testing Procedure for MCAUIndiana State Pipe Trades (ISPTA) Identification Card <br />Your name has been randomly picked by MCAT/ISPTA computer generated selection <br />program to take a drug test as soon as possible in order to update your MCAI/ISPTA <br />identification card. Our computer program selects the names of one -twelfth (1/12) of all <br />participant cardholders for update each month. We want to re-emphasize that your selection for <br />a test is purely a random one. You will' be taking the same test, which you initially took when <br />you received your MCAIASPTA card. <br />In order to avoid any potential interruption in the status of your card, you must be tested <br />by (Example date: April 15, 2016). Tests are to be taken on your own time at any one of the <br />facilities identified on the attached list. You will be mailed a $35.00 MCAI/ISPTA expense <br />reimbursement check if your test is negative/valid. Reimbursement checks will be void after 60 <br />days from the date of the check. <br />All .MCAT/ISPTA participants who are working may have their MCAT/ISPTA card <br />verified with the database administrator in order to determine if their card is "valid" or "not <br />valid" under MCAI/ISPTA Program, This procedure will protect everyone's confidentiality. <br />Please remember that if you fail to take the test by the date listed above, the result <br />may be treated as a positive test. <br />**Please be aware of the closing time of the collection facility you choose to report to for your <br />random test. If you start the testing process you will be expected to finish the process (by <br />providing a sufficient urine specimen) within 2 hours OR by the time the <br />facility <br />closes, <br />which ever is sooner. , If you fail to finish the process, it will be considered a refusal to test <br />which has the same consequences as a positive test result.
