12.0 CONSTRUCTION PHASE SERVICES <br />12.1 Review the Contract Information Book (CIB) prepared by INDOT. <br />12.2 Review and respond to Contractor questions submitted during bidding. <br />12.3 Attend one (1) Pre -construction Conference with the responsible agent(s) of the <br />LPA and the CONTRACTOR. <br />12.4 Review shop drawings. <br />12.5 Make services available to LPA during construction of the work for the <br />interpretation of the plans where disagreement may arise and for consultation <br />during construction in the event unforeseen or unusual conditions may arise. <br />13.0 SERVICES NOT BEING PROVIDED BY CONSULTANT <br />Any services not specifically included above are not included in CONSULTANT's <br />Services to be furnished being provided under this Contract. The Services to be <br />furnished not being provided by CONSULTANT under this Contract include, but are <br />not limited to, the following: <br />13.1 Subsurface utility engineering. <br />13.2 Stream and wetland mitigation. <br />13.3 Stage 2 Plans. <br />13.4 Right of way acquisition services. <br />13.5 Right of way staking. <br />13.6 Construction inspection. <br />13.7 Individual IDEM Section 401 Water Quality Certification <br />13.8 Individual Section 404 Department of the Army Corps of Engineers Permit. <br />13.9 Providing any type of property surveys and staking to enable contractor(s) to proceed <br />with their work; and providing other special field surveys. <br />13.10 SWCD and MS4 review fees. <br />13.11 SUE services or coordination of SUE services. <br />13.12 Drafting and submission for approval of reimbursable and subordination agreements <br />including review and approval of cost estimates. <br />13.13 Monitoring of accurate placement of utility relocations. <br />13.14 Inspection for Buy America Compliance. <br />13.15 Daily inspections of utility relocation including material inspections. <br />13.16 Right -of -Way Engineering and Land Acquisition Services. <br />13.17 Pavement Design, the standard pavement section will match INDOT standard drawings E <br />502-N V U F-01. <br />13.18 Temporary lighting plans. <br />13.19 Custom foundation design, geotechnical analysis, or subsurface investigations as part of <br />this lighting design. <br />13.20 Signal design services relative to any other upstream or downstream intersections along <br />US Route 20 or Wilber Street. <br />PAGE 10 OF 11 APPENDIX "A" <br />
