3. The following types of records shall be maintained for a minimum period of 1 <br />year. <br />a. Records of all test results below 0.02. <br />Maintenance of Specific Types of Records <br />1. The following types of records related to the collection process shall be <br />maintained: <br />a. Collection log books, if used. <br />b. Calibration documentation for EBT devices. <br />C. Documentation of BAT training. <br />d. Documents generated in connection with decisions to administer <br />reasonable suspicion alcohol tests. <br />e. Documents generated in connection with decisions to administer <br />reasonable suspicion alcohol tests. <br />f. Documents verifying existence of a medical explanation of the inability of <br />a covered employee to provide adequate breath for testing. <br />2. The following types of records related to test results: <br />a. Trisco Systems, Inc.'s copy of the alcohol test form, including the results <br />of the test. <br />b. Documents related to the refusal of any covered employee to submit to a <br />required alcohol test. <br />C. Documents presented by a covered employee to dispute the result of an <br />alcohol test administered under the AMPP. <br />3. Records related to other violations outlined in the AMPP.. <br />4. The following types of records related to referrals and evaluations: <br />a. Records pertaining to a determination by a substance abuse professional <br />concerning a covered employee's need for assistance. <br />b. Records concerning a covered employee's compliance with eh <br />recommendations of the substance abuse professional. <br />5. Records related to the Trisco Systems, Inc.'s MIS annual alcohol misuse testing <br />data. Trisco Systems, Inc. shall submit the required alcohol misuse MIS testing <br />data to FHWA annually as prescribed by the regulations. <br />6. The following types of records related to education and training of employees and <br />supervisors: <br />a. Materials on alcohol misuse awareness, including a copy of Trisco <br />Systems, Inc.'s policy on alcohol misuse. <br />b. Documentation of compliance with the requirements of 49 CFR Part <br />382.601 (Employer policy requirements). <br />C. Documentation of training provided to supervisors for the purpose of <br />qualifying the supervisors to make a determination concerning the need <br />for alcohol testing based on reasonable suspicion. <br />d. Certification that any training conducted under the AMPP complies with <br />the requirements of 49 CFR Part 382 and 40. <br />54 <br />
