SAP will perform an initial evaluation, recommend any additional treatment if necessary, and <br />refer employees needing assistance for treatment covered under their health insurance program. <br />The SAP who determines that a covered employee who requires assistance in resolving problems <br />associated with alcohol misuse will not refer the employee to the SAP's private practice or to a <br />person or organization from which the SAP receives remuneration or has a financial interest. <br />(This does not prohibit a SAP from referring an employee for assistance to a public agency; a <br />person under contract with Trisco Systems, Inc. to provide treatment for alcohol on behalf of <br />Trisco Systems, Inc.; the sole source of therapeutic treatment under the employee's health <br />insurance policy; or sole source of treatment reasonable accessible to the employee.) <br />Recordkeeping Procedures <br />The Trisco Systems, Inc., APM (Alcohol Program Manager) or designee shall maintain the <br />alcohol testing records in accordance with the provisions set out in the AMPP. Records shall be <br />maintained for the specified periods of time as required in 49 CFR Parts 382 and 40. <br />Record Retention Provisions <br />I . The following types of records shall be maintained for a minimum period of 5 <br />years: <br />a. Records of employee alcohol test results with results indicating an alcohol <br />concentration of 0.02 or greater. <br />b. Documentation of refusals to take required alcohol tests. <br />C. Calibration documentation of each EBT used in alcohol testing, including <br />records of the results of external calibration checks. <br />d. Employee evaluation and referrals. <br />e. Management Information System (MIS) annual alcohol misuse report data <br />(if required). <br />f. Documents pertaining to "missed tests". (Missed test = a test that is not <br />conducted within 8 hrs. "Missed test" information shall be submitted to <br />OPS, (when the MIS information is submitted) for three years beginning <br />in 1995. <br />2. The following types of records shall be maintained for a minimum period of 2 <br />years. <br />a. Records related to the collection process (except calibration of EBT <br />devices). <br />b. Records related to training. <br />C. Records of the inspection and maintenance of each EBT used in employee <br />testing. <br />d. Documentation of Trisco Systems, Inc.'s compliance with the QAP for <br />each EBT it uses for alcohol testing under the AMPP. <br />e. Records of the training and proficiency testing of each BAT used in <br />employee testing. <br />f. Log books used in conjunction with EBTs. <br />53 <br />
