<br />COA Application Project Overview <br />Home Owner: Tom Keller <br />Address: 1321 E Wayne St S <br /> South Bend, IN 46615 <br />Work Type: Shingle Roof Replacement (House Only) <br />Existing Condition <br /> The existing roof is in poor condition and in need of replacement. <br />Materials <br /> The roof currently has an architectural shingle on it. These shingles will be removed and <br />disposed of a long with the existing drip edge and gutter apron metals. The existing substrate is unknown <br />currently. If the existing substrate does not consist of 4’ x 8’ plywood or OSB sheeting, then Millwood <br />will install 4’ x 8’ x 7/16” OSB sheets overtop of the deck boards so that the new shingles will have a <br />more suitable surface to attach to. We will also be investigating the underside of the roof deck in order to <br />potentially address a heat loss/ice damming issue on the house. 3 ft. of ice and water shield will be <br />installed on all eaves and the entire roof deck will receive synthetic felt underlayment. New “white” drip <br />edge and gutter apron metal will be installed around the entire perimeter of the roof. New CertainTeed <br />Landmark shingles will then be installed in the color “Georgetown Grey”. A ridge vent will be installed <br />underneath of the ridge cap shingles as well for ventilation. The existing “white” aluminum siding will be <br />replaced with new CertainTeed Vinyl Cedar Impressions Double 7” Straight Edge Perfection Shingles 3G <br />in the color “Flagstone Gray”. We also will be replacing the one existing casement/circle top window <br />with a new Provia Endure Slider/Circle Top vinyl window (white interior/white exterior). We will also be <br />replacing a small amount of white soffit with with new white vented soffit. We will install ventilation <br />shoots in the eave/attic space to allow for better air flow at this time as well. <br />Scale/Dimensions <br /> The size of the roof is approximately 18 square or 1800 square feet. <br />Alterations <br /> No alterations will be made to the structure or its materials a part from what has been <br />previously mentioned. <br /> <br />Respectfully Submitted, <br />Jack Reed - Estimator
