The Provider must be certified and meet the requirements of IC 4-13.6-4 and the RFQ submitted must include a copy of the firm’s certification of qualification issued under IC 4-13.6-4. <br /> Bowen is certified for Professional Services, including energy audits, and for the construction <br />of utility infrastructure projects. These two certifications meet the intent of IC 4-13.6-4. Identify Professional Engineers directly employed by the Provider responsible for this project. Include License number of the person responsible for the project. <br />Doug Stout is the Professional Engineer (IN 10303062) responsible for this project. Bowen has 16+ <br />additional Licensed Professional Engineers on staff. <br /> viii. Describe if the Provider is a manufacturer, contractor, or engineering firm. Clearly delineate what services or projects the Provider is including from their own company as a part of the project. <br />In addition to being a Qualified Provider, Bowen is a self-performing water and wastewater <br />contractor. It is our intention to not only self-perform 100% of the Provider and General <br />Contractor duties, but also self-perform the construction work within our capabilities. For <br />your project, we expect to self-perform site work, concrete, process mechanical piping, <br />and process equipment installation. We will accomplish this through our project managers, <br />project engineers, superintendents, foremen, carpenters, operators, pipe-fitters, millwrights, <br />boilermakers, iron workers and laborers. <br /> <br />COMPANY BACKGROUND & QUALIFICATIONS
