Version 6-8-2017 <br />18 <br />APPENDIX "A" <br />SERVICES TO BE FURNISHED BY CONSULTANT: <br />In fulfillment of this Contract, the CONSULTANT shall comply with the requirements of the appropriate regulations <br />and requirements of the Indiana Department of Transportation and Federal Highway Administration. <br />The scope of this project includes survey and design for sidewalk and curb ramp reconstruction in the area around <br />Kennedy Academy. The area is generally bounded by Lincolnway West to the north, Bendix Drive to the west, <br />Bendix Drive and Orange Street to the south, and Birdsell Street to the east, in the City of South Bend, Indiana. <br />Within this area, there are segments of missing, heaved, or deteriorated sidewalk. An assessment will be completed <br />to evaluate the existing condition of sidewalk throughout the project area and provide recommendations for sidewalk <br />construction/reconstruction prioritization. Based on planning level estimation, it is anticipated that up to 12,000 LFT <br />of 5-foot wide sidewalk and corresponding curb ramps can be constructed with the $2,000,000 in federal and local <br />funding that has been awarded. Curb, driveway approaches, and pavement patching will be constructed when <br />necessary to construct the sidewalk and curb ramps. <br />This project is not intended to correct roadway deficiencies, such as drainage, pavement restoration, pavement <br />markings or signage. Additionally, it is not the intent of this project to address minor ADA deficiencies, such as <br />longitudinal or cross slope of sidewalk and curb ramps, unless the deteriorated condition of the sidewalk or curb <br />ramp also warrants replacement. <br />The CONSULTANT shall be responsible for performing the following activities: <br />A.Project Management & Coordination <br />The CONSULTANT will follow the INDOT Local Public Agency (LPA) Guidance Document, and <br />procedures therein. <br />1.The CONSULTANT shall manage the activities described in Appendix “A” (Scope of Work) <br />among the project team members. The CONSULTANT’s project manager will be in charge of <br />overall project management in addition to being the LPA’s direct link to the design team for issues <br />concerning administration and technical execution. He or she will be responsible for coordinating <br />the activities of all SUBCONSULTANTs and acting as liaison between the LPA and any outside <br />agencies and/or groups concerning the Project. The project manager will also be responsible for <br />maintaining schedule and budget. <br />2.The CONSULTANT shall coordinate and attend meetings with the LPA, as appropriate, to <br />facilitate the management of the project design elements. Meetings will be held to discuss <br />technical issues requiring action by the LPA and other third parties. Meetings will be held with <br />the LPA on an as-needed basis to facilitate the acquisition of the necessary project right-of-way <br />parcels. The CONSULTANT shall coordinate and prepare MACOG quarterly reports for review <br />by the LPA prior to submittal to MACOG and attend the quarterly update meetings. <br />3.The CONSULTANT shall implement quality management and control procedures, to satisfy the <br />project requirements. The CONSULTANT shall coordinate these procedures with the <br />SUBCONSULTANTs at each plan submittal stage for continuity of design standards and plans <br />throughout the project limits. <br />B.Sidewalk Priority Assessment <br />1.The LPA has contracted another firm to perform a City-wide sidewalk assessment. It is anticipated <br />this assessment will be completed by July 1, 2023. The information will provide a rating of the <br />existing sidewalk conditions and will identify areas where sidewalk does not exist. <br />2.The CONSULTANT shall provide engineering services to develop a Sidewalk Priority <br />Assessment, which shall evaluate the sidewalk conditions within the project area and establish <br />recommendations for sidewalk replacement or construction of new sidewalk.
