19 <br />APPENDIX "A" <br />SERVICES TO BE FURNISHED BY CONSULTANT: <br />In fulfillment of this Contract, the CONSULTANT shall comply with the requirements of the appropriate <br />regulations and requirements of the Indiana Department of Transportation and Federal Highway Administration. <br />The CONSULTANT shall be responsible for performing the following activities: <br />A.The OWNER desires to develop a Safe Routes to School sidewalk and ADA curb ramp upgrades <br />adjacent to Muessel Primary and Holy Cross Schools. The improvements shall be developed in <br />the current right of way. The scope of this project shall include survey, design, environmental <br />documentation, cultural resources, utility coordination, and construction administration. <br />B.The CONSULTANT will conduct a site analysis and survey for preparation of design plans in <br />conformance with the requirements of the Indiana Department of Transportation Design Manual. <br />Survey to include: <br />1.A survey along the proposed safe routes to the schools. <br />2.The survey will include enough shots along curbs, sidewalks, driveways, and curb ramps <br />to complete our design within INDOT and ADA standards. <br />3.Survey of improvements including roads, sidewalks, curb ramps, curb, driveways, <br />utilities, sign/poles, individual trees and other structures as needed. <br />4.Location of utilities within the project area. <br />5.Location of property lines and easements within the project area. <br />C.Conformity to the National Environmental Policy Act (NEPA) of 1969 for this project will be <br />completed in accordance with INDOT’s most recent Categorical Exclusion (CE) Manual dated <br />February 1,2021. The environmental documentation for this project shall require a CE Level -1 to <br />be reviewed and approved by INDOT Central Office. Major tasks associated to this include the <br />following: <br />1.Project Organization: This task includes all file management and general project <br />management activities. <br />2.Prepare Material to Append to Level-1 CE: This task includes the preparation, revisions, <br />approval, and distribution of the documentation necessary for review of the Level-1 CE. <br />3.Documentation: Prepare requested digital copies of approved CE and Section 106 <br />documentation for INDOT distribution and the OWNER’s files. <br />4.Red Flag Investigation. This task includes review and approval by INDOT Site <br />Assessment and Management (SAM) Team. <br />5.USFWS IPaC coordination. This task assumes a finding of “Not Likely to Adversely <br />Affect” upon threatened and endangered bat species. A “Likely to Adversely Affect” <br />finding will constitute additional services. <br />6.Early Coordination. Correspondence with various required local, state, and federal <br />agencies. <br />7.Regulated Waters Determination. This task involves the investigation for potential <br />regulated streams and wetlands. This task assumes no such features are present, and a <br />formal “Regulated Waters of the U.S.” report will not be necessary. Preparation of such <br />a report will constitute additional services. <br />8.Cultural Resource Investigation. <br />a.As currently scoped, this project will require a full Section 106 investigation. <br />Tasks associated to this include: <br />i)Cultural resource agency coordination. <br />ii)Documentation of the Section 106 finding for review and approval by <br />INDOT and the State Historic Preservation Officer.
