Version 6-8-2017 <br />29 <br />APPENDIX "B" <br />INFORMATION AND SERVICES TO BE FURNISHED BY THE LPA: <br />The LPA shall furnish the CONSULTANT with the following: <br />1.Designate in writing a person to act as LPA's representative with respect to the services to be <br />rendered under this Agreement. Such person shall have complete authority to transmit instructions, <br />receive information, interpret and define LPA's policies and decisions with respect to the <br />CONSULTANT's services for the Project, <br />2.Criteria for design and details for signs, signals, lighting, highway and structures such as grades, <br />curves, sight distances, clearances, design loading, etc., <br />3.Standard Specifications and standard drawings applicable to the project, <br />4.All written views pertinent to the location and environmental studies that are received by INDOT, <br />5.Traffic Signal Warrants (New Signal) and Traffic Lighting Warrants (New Lighting), <br />6.Available data from the transportation planning process, <br />7.LPA utility plans including the location of traffic signal conduits throughout the affected areas, <br />8.Aerial Survey information, <br />9.Existing water quality data, <br />10.Previous Geotechnical investigation(s), if applicable, <br />11.Arrange for a venue to hold public meetings, if required, <br />12.Arrange for access to and make all provisions for CONSULTANT and/or Subcontractors to enter <br />upon public and private property as required to perform services under this Agreement, <br />13.Provide legal services for condemnation proceedings, if required, <br />14.The money and proof of payment/delivery for all payments due property owners and/or applicable <br />property interests, <br />15.Partial mortgage release fees, <br />16.Property Management, <br />17.Conveyance instruments which have been prepared and/or approved for each parcel by LPA legal <br />counsel, <br />18.Furnish approvals and necessary permits from all governmental authorities having jurisdiction over <br />the Project and such approvals and consents from others as may be necessary for completion of <br />the Project. Permit fees shall be paid by LPA at time of submission of said applications, <br />19.Examine all studies, reports, sketches, drawings, specifications, proposals and other documents <br />presented by CONSULTANT, obtain advice of attorney, insurance counselor and other consultants <br />as LPA deems appropriate for such examination and render in writing decisions pertaining thereto <br />within a reasonable time so as not to delay the services of CONSULTANT, <br />20.Provide written approval of completed work phases as described in Appendix "A" of this <br />Agreement. Accomplish reviews and provide written approvals in a timely manner, <br />21.Furnish, or direct CONSULTANT to provide, Additional Services as stipulated in Appendix ā€œEā€ <br />of this Agreement or other services as required, <br />22.A location for the storm water system outfall, constructed prior to this project, <br />23.Existing/as-built traffic signal plans for the intersections of Bendix Drive/Lathrop Street and <br />Bendix Drive/Voorde Drive.
