Version 6-8-2017 <br />18 <br />APPENDIX "A" <br />SERVICES TO BE FURNISHED BY CONSULTANT: <br />In fulfillment of this Contract, the CONSULTANT shall comply with the requirements of the appropriate regulations <br />and requirements of the Indiana Department of Transportation and Federal Highway Administration. <br />The scope of this project includes survey and design for road reconstruction along Bendix Drive, between Lathrop <br />Street and Voorde Drive, in the City of South Bend, Indiana. The current 4-lane roadway is to be reduced to a 3-lane <br />section and a multi-use path will be constructed starting just south of the intersection with Voorde Drive to <br />approximately 480 feet north of the intersection with Lathrop Street, for a length of approximately 4,100 feet. <br />Additionally, this will require reconstructing Lathrop Street from approximately 150 feet west to 150 east of the <br />centerline of Bendix Drive, and approximately 150 feet of Voorde Drive, for a total S-Line length of approximately <br />450 feet. Bendix Drive is anticipated to be reconstructed as a 38-foot-wide concrete roadway with curb and an 8- <br />foot-wide concrete multi-use path along the east side of the corridor. Along with reconstruction of the intersections, <br />new traffic signals, with mast arms, at Lathrop Street and Voorde Drive will be installed. Currently, a combined <br />sanitary and storm sewer exists through a portion of the proposed project. The sewers will be separated with the <br />installation of a new storm sewer and reuse of the existing combined sewer for sanitary sewer flow. Only minor <br />improvements to the existing sanitary/combined manholes are anticipated. It is anticipated that the LPA will have <br />constructed an outlet for the proposed storm sewer system along Bendix Drive, prior to this project. <br />Additionally, the existing water main will be replaced. Approximately 4,100 feet of existing 12-inch water main will <br />be removed or retired in place along Bendix Drive, and a new 12-inch water main and appurtenances will be installed. <br />The water main will connect to the existing 10-inch and 6-inch water main on Voorde Drive and an existing 12-inch <br />water main on Lathrop Street. As part of this work, all lead service lines (up to five lead service lines) shall be <br />replaced to the water meter per Title 40, Chapter I, Subchapter D, Part 141, Subpart I in the Code of Federal <br />Regulations. It is anticipated that the storm sewer and water main will cross the NICTD railroad via jack and bore <br />method. Utility coordination, Subsurface Utility Engineering and Geotechnical Investigation are also included as <br />part of this project. <br />The CONSULTANT shall be responsible for performing the following activities: <br />A.Project Management & Coordination <br />The CONSULTANT will follow the INDOT Local Public Agency (LPA) Guidance Document, and <br />procedures therein. <br />1.The CONSULTANT shall manage the activities described in Appendix “A” (Scope of Work) <br />among the project team members. The CONSULTANT’s project manager will be in charge of <br />overall project management in addition to being the LPA’s direct link to the design team for issues <br />concerning administration and technical execution. He or she will be responsible for coordinating <br />the activities of all SUBCONSULTANTs and acting as liaison between the LPA and any outside <br />agencies and/or groups concerning the Project. The project manager will also be responsible for <br />maintaining schedule and budget. <br />2.The CONSULTANT shall coordinate and attend meetings with the LPA, as appropriate, to <br />facilitate the management of the project design elements. Meetings will be held to discuss <br />technical issues requiring action by the LPA and other third parties. Meetings will be held with <br />the LPA on an as-needed basis to facilitate the acquisition of the necessary project right-of-way <br />parcels. The CONSULTANT shall coordinate and prepare quarterly TTAC forms for review by <br />the LPA prior to submittal to MACOG and attend the quarterly TTAC meetings. <br />3.The CONSULTANT shall implement quality management and control procedures, to satisfy the <br />project requirements. The CONSULTANT shall coordinate these procedures with the <br />SUBCONSULTANTs at each plan submittal stage for continuity of design standards and plans <br />throughout the project limits.
