Additional <br />operation and maintenance and/or additional pretreatment is required to meet the Pretreatment <br />Standards and Requirements.” <br /> <br />b. If laboratory analytical data was not reported on the PCR, the requirement for the PCR to be <br />signed by a Certified Industrial Wastewater Operator is not required, unless otherwise indicated <br />by the Director. <br /> <br />4. All reports, including but not limited to PCRs, the Flow Measurement Plan, the Slug Control Plan, and <br />corrective actions in response to permit violations, shall include the following certification statement <br />signed by an Authorized Representative. See Part IV (B)(15) of this permit for requirements concerning <br />the Authorized Representative. <br /> <br />“I certify under penalty of law that this document and all attachments were prepared <br />under my direction or supervision in accordance with a system designed to assure that <br />qualified personnel properly gather and evaluate the information submitted. Based on <br />my inquiry of the person or persons who manage the system, or those persons directly <br />responsible for gathering the information, the information submitted is to the best of my <br />knowledge and belief, true, accurate, and complete. I am aware that there are significant <br />penalties for submitting false information, including the possibility of fine and <br />imprisonment for knowing violations.” <br /> <br />a. If the designation of an Authorized Representative is no longer accurate because a different <br />individual or position has responsibility for the overall operation of the facility or overall <br />responsibility for environmental matters for the company, a new written authorization must be <br />submitted to the Director prior to or together with any reports to be signed by the Authorized <br />Representative. Please see Part IV B. 15. of this permit for more information on designating a <br />new Authorized Representative. <br /> <br /> <br />B. Additional Sampling <br /> <br />1. If the permittee monitors their wastewater discharge for any pollutant more frequently than required by <br />this permit, using test procedures in 40 CFR 136 or amendments thereto or otherwise approved by U.S <br />EPA or as specified in the permit, the results of such monitoring shall be included in any calculations of <br />actual daily maximum or monthly average pollutant discharge, and results shall be reported in the specific <br />time frame report submitted to the Control Authority. <br /> <br />C. Notification of Batch or Infrequent Discharge <br /> <br />1. Within 30 days of the effective date of this permit, the permittee shall commence to notify the City of <br />South Bend via e-mail twenty-four (24) hours in advance of all batch or infrequent discharges to the <br />City’s sewer so that during this time compliance monitoring may be conducted. The subject line shall <br />include the words “discharge notification” followed by the outfall number, anticipated time of discharge, <br />and the anticipated volume of discharge. The permittee may request a modification of notification <br />requirements. All requests for modification of notification requirements are subject to approval by the <br />City of South Bend. The notification shall be made via e-mail to Pretreatment@SouthBendIN.Gov. <br />
