The following conventional pollutants shall incur a surcharge in excess of the stated limitations <br />below, as stated in Section 17-23(b) of the City of South Bend’s Municipal Code. <br /> <br /> Surcharge for ammonia (NH3-N) >25 mg/L <br /> Surcharge for BOD5 >250 mg/L <br /> Surcharge for phosphorus >10 mg/L <br /> Surcharge for Total Suspended Solids (TSS) >250 mg/L <br /> <br /> <br />C. Representative Sampling <br /> <br />1. Sampling shall be carried out by customarily accepted methods to reflect the effect of <br />constituents upon the sewage works and to determine the existence of hazards to life, limb and <br />property. <br /> <br />2. All wastewater samples must be representative of the permittee’s discharge. <br /> <br />3. Wastewater monitoring and flow measurement facilities shall be properly operated, kept clean, <br />and maintained in good working order at all times. <br /> <br />a. The failure of a permittee to keep its monitoring facility in good working order shall not <br />be grounds for the permittee to claim that sample results are unrepresentative of its <br />discharge. <br /> <br /> <br /> <br />PART III- REPORTING REQUIREMENTS <br /> <br />A. Monitoring Reports <br /> <br />1. Monitoring results obtained shall be summarized and reported on an Industrial User Periodic Compliance <br />Report (PCR). The PCRs are due on the twentieth (20th) day of the month following the required <br />monitoring time frame. <br /> <br />2. The PCR shall indicate the nature and concentration of all pollutants in the effluent for which sampling <br />and analysis were performed during the monitoring time frame preceding the submission of each report, <br />including measured flows. The PCRs shall also include all supporting documentation necessary to <br />demonstrate compliance. <br /> <br /> <br /> <br />3. The PCR shall be signed by a Certified Industrial Wastewater Operator of the State of Indiana indicating <br />whether Pretreatment Standards and Requirements are being met on a consistent basis, and if not whether <br />additional operation and maintenance and/or additional pretreatment is required to meet the Pretreatment <br />Standards and Requirements. <br /> <br />a. The Certified Operator shall sign one of the following statements: <br /> <br />“Pretreatment Standards and Requirements are being met on a consistent basis.”
