employee having a physical injury that requires immediate medical attention away from the work <br />site will be required to submit to drug and alcohol tests. <br />The drug test for a Post -Accident incident will be composed of a five panel urine test. The drugs <br />that will be tested for under Non -DOT include marijuana (THC), cocaine (COC), opiates (OPI), <br />amphetamines (AMP), and phencyclidine (PCP.) <br />Return -to -Duty Testing <br />Under TGB UNLIMITED, INC. OBA SIT BANCROFT ELECTRIC's policy, before an employee is <br />considered for reinstatement after having engaged in prohibited conduct (Section H) which <br />constitutes a violation of TGB UNLIMITED, INC. OBA S/T BANCROFT ELECTRIC's policy, the <br />employee must provide a negative Retum-to-Duty drug and/or alcohol t est. <br />This test must be completed after an assessment by the company's EAP Provider. <br />The Return -to -Duty test is not limited to a specific substance (i.e. the drug for which the <br />employee tested positive). Additionally, a drug test may be performed in conjunction with an <br />alcohol test <br />depending on the EAP Provider's assessmen.t The result of the alcohol test must be less than <br />0.02. <br />If an employee refuses to submit to a Return -to -Duty test orfails to appear at the collection site <br />within a reasonable time, as determined by TGB UNLIMITED, INC. OBA S/T BANCROFT ELECTRIC <br />and after being directed to do so bythe DER, this is considered a "Refusal to Test". The employee <br />will not be permitted to return to their position. <br />The cost of a Ret urn -t o -Duty test shall be paid by the employee. <br />Follow -Up Testing <br />Under TGB UNLIMITED, INC. DBA S/T BANCROFT ELECTRIC's policy, before an employee is <br />considered for reinstatement after having engaged in prohibited conduct (Section H) which <br />constitutes a violation of TGB UNLIMITED, INC. DBA S/T BANCROFT ELECTRIC's policy, the <br />employee must be assessed by the company's EAP Provider and undergo frequent unannounced <br />Follow-up urine and/or breath testing following their Return -to -Duty Test. The employee will <br />remain in the random testing program. The company's EAP Provider & the employer will <br />determine the frequency and duration of the follow-up tests. <br />The employer or their Third Party Administrator (TPA) will schedule the test dates and ensure <br />that the appropriate tests are conducted. <br />Tests may be for both controlled substances and/or alcohol regardless of whether the prohibit <br />ion violation concerned either specific substance. There is no limit to the number and <br />frequency of the follow-up tests. <br />The cost of Follow-up testing requirements shall be paid by the employee <br />15 <br />
