Safeguards for Controlled Substance Testing <br />o The Collector must complete mandated qualification training as defined in 49 CFR Part 40 <br />to be able to perform collections. <br />o The Collector must obtain photo identification from the employee or be identified by an <br />TGBUNLIMITED, INC. DBA SIT BANCROFTELECTRIC representativepriortoadministering <br />the test. <br />o The Collector will ask the employee to display the contents of their pockets prior to the <br />test being taken. <br />o The Collector will ask the employee to wash their hands only with water or a moist <br />towelette. <br />o The Collector and employee will check the identification numbers of the seals with the <br />number on the CCF. <br />o The Collector will ask the employee to sign the CCF signifying the correctness of the data <br />for testreporting. <br />o The Collector will unwrap and wrap the specimen containers in the presence of the <br />employee. <br />o After the specimen is collected, the Collector will inspect the specimen for sufficient <br />volume, temperature and signs of tampering . <br />o The specimen sample containers are placed in a shipping container, sealed with tamper <br />proof seals in the presence of the employee . The employee will be asked to initial the <br />seals on the specimen sample containers. <br />o The Laboratory will check the seals on the specimen bottles upon receipt to ensure they <br />are not broken, if they are broken, the Laboratory will report the test as cancelled. <br />o The Laboratory will report the results to the MRO by electronic communication. <br />Direct Observation Collection is Mandatory in the Following Circumstances <br />o The Laboratory reports an invalid specimen / invalid result and the MRO reports there <br />was not an adequate medical explanation for the results. <br />o The test results are reported as positive adulterated or substituted cancelled with no split <br />specimen sample available. <br />o The Collector observed materials brought to the collection site orthe employee's conduct <br />clearly indicated an attempt to tamper with the specimen. <br />o The Collector notes that the temperature of the specimen was out of range or the <br />specimen appeared to have been tampered with. Note: The Collector is NOT required to <br />take the employee's body temperature. The DER from TGB UNLIMITED, INC. DBA SIT <br />BANCROFT ELECTRIC will be notified of the out of temperature specimen after the direct <br />observation test is completed. <br />o The MRO reports a negative -dilute result with a creatinine concentration greater than or <br />equal to 2 mg/dL, but less than or equal to 5 mg/dl. <br />The observer must physically watch the donor urinate into the collection container. The <br />observer must be the same gender as the employee. If someone else is to observe the <br />collection other than the collector in order to ensure a same gender observer, the observer must <br />verbally instruct that person to follow procedures noted below. <br />
