<br />Version 08/25/2021 Page - 11 Public Works Quote <br />2. 5” CONCRETE SIDEWALK – REMOVE & REPLACE <br /> <br />A. General Description <br /> <br />Work for this item includes, but is not limited to, the furnishing of all material, equipment, labor, <br />compaction, subgrade preparation, aggregate, concrete, reinforcing steel, formwork, lines and grades, <br />site restoration, lawful removal and disposal of materials and all other necessary items required to <br />provide the reinforced concrete sidewalk as shown in the plans and as specified herein. <br /> <br />B. Preliminary Work <br /> <br />The Contractor shall verify the locations of all utilities by contacting Indiana 811 at 1-800-382-5544 at <br />least three (3) working days, not counting Saturdays, Sundays or federal and state holidays before <br />proceeding with construction. It shall also be the Contractors responsibility to contact any private or <br />other utility that is not contacted by Indiana 811 and verify the utility locations. <br /> <br />C. Demolition Procedures <br /> <br />The Contractor shall preserve and protect all surrounding property, structures, tenants, visitors, <br />pedestrians, motorists and their property from damage caused by the Contractor’s operations. All <br />demolition activities shall be undertaken in such a way as to avoid any impact to adjacent properties <br />and public right-of-way that are to remain and/or be protected. The Contractor shall repair any damages <br />to the public right of way or private property not included in this demolition as a result of the Contractor’s <br />operations at his/her own expense. <br /> <br />Any falling construction debris and temporary accumulation of trash resulting from demolition activities <br />shall be contained within the subject property lines until promptly removed in a lawful manner. Any <br />debris that falls within adjacent property lines and right-of-way shall be removed immediately. Site <br />restoration shall be completed in such a way as to avoid impact to adjacent properties and right-of-way, <br />including storm water drainage. Any unauthorized damage to adjacent property and/or right of way <br />shall be restored immediately at the Contractor’s expense, and to the Owner’s satisfaction. <br /> <br />Removal of existing sidewalk shall be completed in accordance with Section 202.05 of the INDOT <br />Standard Specifications for Removal of PCCP, Sidewalks, Curbs, RCBA, and Reinforced Concrete <br />Moment Slabs. <br /> <br />D. Concrete <br /> <br />All concrete shall meet the requirements of INDOT Standard Specification section 702 for Class ‘A’ <br />concrete with a minimum 6% entrained air. The Contractor shall provide job mix formula information, <br />including all concrete components and admixtures to the Engineer, sufficiently in advance of the work <br />to allow for review and approval or rejection. The approved job mix formula information shall be written <br />on the first concrete load ticket provided at the site prior to the start of each day's pour for verification. <br /> <br />Reinforced concrete sidewalk shall be constructed in accordance with City of South Bend Standard <br />Drawing 2-28. <br /> <br />E. Placement & Curing <br /> <br />The Contractor shall be responsible for taking all necessary precautions, such as pouring early in the <br />day, providing a watchman after hours, etc., to avoid vandalism or other damage to the concrete during <br />the curing period of the concrete. <br /> <br />The Contractor shall perform all work in accordance with all applicable INDOT Standard Specifications, <br />including but not limited to the specifications for subgrade and subbase compaction and grading, <br />forming, pouring, screeding, tining, curing, etc.. <br />
