(a)
Company Name
Contact Phone:
Contact Name:
Email:
Address: ------------------------- ------------------ City/State/ZIP:
(b)
Name:
Contact Phone:
Qualifications: ---- -------------------------------------------------
Name: Contact
Qualifications: ------ --_---- --_________________ --.
Name: ---------------------------------------------- Contact
Qualifications: __—__-- -------------- —____ —.
Section G - Contingency and Strategic Planning
For each of the following, please provide detailed descriptions. If you run out of space, attach a response to this
application submission.
Emergency Safety Plan - This plan should include, but is not limited to:
o The number of Indiana Law Enforcement Academy certified officers, fire, and emergency
medical personnel, and the need to use any of the City's public safety or emergency response
services.
o If hiring a security service, provide contact information and the number of hired event personnel.
o Proposed internal communications systems and public address systems.
We will have volunteers riding throughout the bike course to ensure that no child has fallen, or
to assist if they have. We will have a first aid tent on site and 2 golf carts, in case anyone feels
unable to continue running or riding their bike. We will have trained life guards in each lane of
the pool, as well as one on deck to monitor the swimming portion of the event.
• Proposed Cleanup Plan - This plan should include, but is not limited to:
o Measures in place to collect and remove trash, litter and recyclables.
We will monitor the event prior and if there is extreme weather, we would cancel the race the
morning of the event. If unexpected weather occurred (i.e. thunderstorms, tomado) during the
race, we would make an announcement through Boomer for parents/gaurdians to meet their
child and either go directly to their vehicle of the bathrooms. All of our staff working the routes
will have radios, so we can quickly communicate and ensure that everyone of the different
courses is notified and safe.
