L FAVAVA <br /> APPENDIX A <br /> SCOPE OF ADDITIONAL SERVICES <br /> South Main Street to Lafayette Connector <br /> Our Design Agreement was originally based upon our proposal dated August 9, 2010. with the <br /> Agreement executed on August 23, 2010. Since that time the Agreement has been <br /> supplemented on three times. It was modified two times to add (1) RIW acquisition services <br /> and (2) relocation agent services. The third supplemental increase (Submitted on October 12, <br /> 201 1) covered the change in design from the construction of the first project of 3-Phases into <br /> one project with revised alignments, and additional alignments and traffic signals replacing the <br /> initially planned roundabout. At the time of the third Supplemental, increased design fees were <br /> projected to carry the project through Construction Contract letting and begin construction. <br /> However additional design and coordination tasks have been requested that are beyond the <br /> vision available at that time. <br /> It is noteworthy to mention that the existing agreement in-place at the time of the first planned <br /> contract letting on May 29, 2012 would have substantially covered our increased design costs. <br /> However a prudent decision made at that time (to delay the contract letting and construction <br /> until 2013) by the City and its design team was instrumental in saving user costs and very likely <br /> construction change orders due to wintering the project. This additional time resulted in several <br /> additional survey, design and coordination tasks including plan revisions to facilitate and <br /> accommodate RIW acquisition changes, additional street lighting, and a separate demolition <br /> project. Collectively these actions will expedite and enable a smooth construction project. <br /> In September 2012 we prepared and submitted Construction Documents a second time, <br /> however funding changes delayed the project and a new Policy of Communication Outreach <br /> was launched by the City which affects the project delivery by adding a new step and bid item. <br /> We anticipate continued involvement with this initiative through the construction project to be <br /> accomplished in 2013. <br /> A significant amount of effort is being expended to develop Communication Outreach by making <br /> design changes that improve the public's appreciation and acceptance of the project by listening <br /> and reacting to individual property owners (both already acquired from and those yet to be <br /> acquired). Meeting with and satisfying the individual property owners most directly impacted by <br /> the project resulted in a higher satisfaction with the process, the project and the City overall. <br /> The following items are requested as effort not envisioned in our October 2011 Supplemental <br /> Request: <br /> Additional Survey Items <br /> • Additional field survey. <br /> o Field layout of the proposed design to facilitate plan interpretation. <br /> • Preparation of additional easements and parcel plat revisions. <br /> • Supplemental title information requests. <br /> December, 2012 Page 1 Project File No. 9022.14 <br />
